摘　　要：目的:正确认识肝总动脉瓣膜单方面闭塞并探讨其介入治疗肝癌的临床价值。材料与方法:肝总动脉闭塞伴胰动脉弓侧支供养肝脏的患者17例,行肠系膜上动脉或胰十二指肠下动脉造影以了解有无肝总动脉瓣膜状闭塞。结果:12例患者肝总动脉呈瓣膜状闭塞,即腹腔动脉造影时肝总动脉完全闭塞,肠系膜上动脉或胰十二指肠下动脉造影时,肝总动脉"闭塞段"血流通畅,类似静脉瓣状。结论:肝总动脉瓣膜状闭塞与多次介入治疗、肝癌切除+术中放置 PCS 及肿瘤浸润有关。放埋置 PCS 时导管不宜过深以保护肝动脉。Purpose:To investigate valve-like occlusion of the common hepatic artery.Materials and Methods:The arteriography of su- perior mesenteric artery(SMA)or inferiar pancreaticocluodened artery(IPDA)was perfomed on 17 patients with liver cancer to observe whether the common hepatic artery was in Valve-lAke occlusion or not.Results:Valve-like occlusion of the common hepatic artery was found in 12 patients:although the common hepatic artery was occlueded completely in celiac arteriogrphy,the'occlude segment'of the common hepatic artery was well patent in arteriography of SMA or IPDA,and seemed to be vein valve.Conclusion:The valve-like occlu- sion of the common hepatic artery was related to the multi-time interventional procedures,PCS implantation during operation,and others such as malignant invasion and atherosclerosis.Attention should be pair to protecting hepatic artery during interventional procedure.
