摘  要:目的构建手术修复的三维眼眶骨折计算机辅助模拟系统(3D-OFCAS).实现眼眶骨折的解剖、诊断分析、手术设计、疗效预测和三维测量。方法系统基于Windows XP,使用Microsoft Visual C++开发平台,借助OpenGL辅助开发丁具构建3D-OFCAS。对1名健康成年男性眼眶进行连续CT扫描,采用优化的MC(Marching cube)算法对CT数据进行三维重构,OpenGL工具进行三维显示与控制。采用基于空间的方法进行三维局部分割,通过像素叠加进行三维量化处理。结果生成适合于手术模拟系统开发的三角面片网格的几何模型。完成具有三维真实感的图像显示、三维截骨以及三维手术模拟功能。实现对健康成年男性眼眶进行三维测量和眼眶体积的计算,左眼眶体积21.07cm^3,有眼眶体积21.05cm^3。结论3D-OFCAS系统为眼眶骨折临床诊断和整形修复手术设计提供了参考。Objective To construct a three- dimensional- orbital fractures computer-aided simulation system (3D-OFCAS)for patients with orbital fracture in reconstructive surgical procedures, and to realize anatomy imagings and dissection, diagnosis analysis, surgery designs, curative effect forecast, three- dimensional (3D) measurement of orbital fracture. Methods The construction of 3D-OFCAS system was based on Windows XP, developed on Microsoft Visual C ++ platform and assisted by development soft OpenGL. Continuous CT scan in orbit was performed for a healthy adult male. In the system, optimized MC (Marching cube)was used in 3D reconstruction of CT data, and 3D display and control were performed by OpenGL. 3D arbitrary region segmentation was performed by spatial segmentation and pixels superimposition was used in 3D measurement. Results A geometry model of triangle surface patch grid suit to surgical simulation system was developed to complete the functions including realistic 3D imagings display, 3D osteotomy, and 3D surgical simulation. Volume calculation and 3D measurement of orbit on the healthy adult male were performed. The left and right orbit volume of this person were 21.07 cm^3 and 21.05 cm^3 respectively. Conclusion The 3D- OFCAS system provides some reference for orbit fracture clinical diagnosis and design of reconstructive surgical procedures.
