用巩膜瓣松解缝线减少青光眼术后低眼压

摘　　要：对４５例５４只难治性青光眼行小梁切除术，术中１次使用０．３ｍｇ／ｍｌ的丝裂霉素Ｃ（ｍｉｔｏｍｙｃｉｎＣ，ＭＭＣ）。其中２３例２８只眼术中采用松解缝线将巩膜瓣紧密缝合，术后５～５５天拆除松解缝线。平均随访时间１１．６个月（７～２６个月）。手术成功率、功能性滤过泡与对照组比较差异无显著性（Ｐ＞０．０５）。浅前房、脉络膜脱离、持续性低眼压与对照组比较差异有显著性（Ｐ＜０．０１，＜０．０５，＜０．０５）。表明本方法能减少手术并发症的发生。Trabeculectomy　were　undertaken　in　45　cases(54　eyes)with　refractory　glaucoma.03mg/ml　mitomycin　C　was　applied　during　surgery.Scleral　flaps　of　23　cases(28　eyes)were　sutured　tightly　with　releasable　suture.The　sutures　were　removed　5　to　55　days　after　operation.The　followup　period　ranged　from　7　months　to　26　months,average　116　months.There　was　no　statistical　significance　in　the　successful　rate　of　the　operation　and　funtional　filtering　blebs　compared　with　the　control　group(P>005)However,there　was　statistical　significance　in　the　shallow　anterior　chamber(P<001),Choroidal　detachment(P<005)and　persistent　low　intraocular　pressure(P<005)compared　with　the　control　group.The　results　indicate　that　this　kind　of　suture　may　decrease　the　occurence　of　the　complication　of　the　filtration　surgery.
