泪道激光在慢性泪囊炎不同时期治疗的临床观察

摘　　要：目的:观察比较伴有慢性泪囊炎、泪道阻塞的炎症期与稳定期应用泪道激光治疗的临床疗效。方法:随机选择230例230眼患有慢性泪囊炎泪道阻塞的患者,分为两组。Ⅰ组90例患者冲出脓性分泌物后马上行泪道激光手术联合局部药物治疗,Ⅱ组140例患者不仅冲出脓性分泌物,且加强抗感染,炎症稳定后再行泪道激光手术联合局部药物治疗,1wk后复查,观察两组疗效,追踪0.5a以上。结果:在Ⅰ组90眼中,痊愈48眼,有效30眼,无效12眼,有效治愈率86.7%,无效率13.3%。在Ⅱ组140眼中,痊愈85眼,有效46眼,无效9眼,有效治愈率93.6%,无效率为6.4%。χ2检验分析两组疗效比较有显著性差异。结论:泪道激光联合局部药物应用在慢性泪囊炎泪道阻塞治疗中有着良好的疗效,而在炎症控制后再行泪道激光手术比在炎症活动期行泪道激光手术疗效更好。AIM：To observe and compare the efficacy after operation of laser assisted dacryocytoplasty for treating lacrimal duct obstruction with chronic dacryocystitis in phase of inflammatory or stable. ·METHODS：Two hundred and thirty cases of lacrimal duct obstruction with chronic dacryocystitis were randomly divided into two groups.90 cases in group Ⅰ were given operation of laser assisted dacryocytoplasty combining local drug remedy after dashing out the pyoid secretion at once;140 cases in group Ⅱ were not only dashed out the pyoid secretion but also given antibacterial drug,then given operation of laser assisted dacryocytoplasty combining local drug remedy after inflammation was stable. All cases under-went callback after a week,and were followed up for more than 6 months. ·RESULTS：For the 90 cases in group Ⅰ,48 cases were cured;30 cases were improved,and 12 cases had no effect,the effective rate was 86.7%,and the ineffective rate was 13.3%. For the 140 cases in group Ⅱ,85 cases were cured;46 cases were improved,and 9 cases had no effect. The effective rate was 93.6%,and the ineffective rate was 6.4%. There were significant differences between group Ⅰ and group Ⅱ by χ2 test. ·CONCLUSION：Laser assisted dacryocytoplasty combin-ing local drug remedy to treat lacrimal duct obstruction with chronic dacryocystitis is effective,and the fulfilment of laser assisted dacryocytoplasty when inflammation was stable seems to be more effective than that when inflam-mation was active.
