IPS-EMPRESSⅡ全瓷贴面对四环素牙颜色改善的初步临床研究

摘　　要：目的研究IPS-EMPRESSⅡ全瓷贴面对四环素牙颜色的改善情况。方法对30例四环素上前牙采用IPS-EMPRESSⅡ全瓷系统进行贴面修复,用试色糊剂模拟粘结后的效果。用电脑比色仪测定基牙唇面中1/3的L*、a*、b*值,修复体自身的L*、a*、b*值以及修复体在基牙上就位后无粘结剂时和采用试色糊剂试粘接时唇面中1/3的L*、a*、b*值。根据公式ΔEa*b*=(ΔL*2+Δa*2+Δb*2)1/2计算色差(ΔEa*b*),单因素方差分析L*、a*、b*值变化。结果修复前后的L*、a*、b*值之间有统计学差异(P<0.05),修复后的L*值增大,a*b*值减小。试粘接前后的L*、a*、b*值之间存在差异,试粘接后L*值和b*值增大,a*值减小,其中采用WOT糊剂试粘接后的L*值和b*值最大,与其余四组相比均有统计学差异(P<0.05)。修复后的L*、a*、b*值与贴面自身相比存在差异,修复后的L*值偏小,a*b*值偏大,其中采用WOT糊剂试粘接后的L*值和采用A3、Trt、WOT和B0.5糊剂试粘接后的b*值与贴面自身的L*值和b*值相比无统计学差异(P>0.05),其余均有统计学差异(P<0.05)。修复前后的最小色差可达9.66NBS。试粘接前后A1组的平均色差最小,为0.68NBS,WOT组的平均色差最大,为1.87NBS。与贴面自身相比,试粘接前的平均色差最大,为2.56NBS,采用WOT糊剂试粘接后的平均色差最小,为0.43NBS。结论IPS-EMPRESSⅡ全瓷贴面可以改善四环素牙的颜色,但修复后最终的颜色易受基牙影响;采用遮色性较强的粘接剂可以改善粘接后的颜色。Objective To evaluate the color change of tetracycline teeth before and after restored with the IPS-EMPRESSⅡ ceramic veneers.Methods Thirty anterior tetracycline teeth were restored with IPS-EMPRESSⅡ ceramic veneers for improving the color. Before permanent adhesion,five types try-in pasters different in color were substituted for the luting agent to represent the final result. The change of color was evaluated by CIEL＊A＊B＊ system and ΔEa＊b＊ （ΔEa＊b＊=（ΔL＊2＋Δa＊2＋Δb＊2）1/2）. An one-way ANOVA was used to analyze the change in L＊a＊b＊ values.Results After veneered,the teeth＇s color had obviously changed with a minimum chromatism of 9.66NBS,the L＊ value increased,and the a＊ and the b＊ value decreased,the differences had statistical significance （P〈0.05）. The color of veneered teeth＇s had differences with the veneers itself,the L＊ values were lower,and the a＊ and the b＊ values were higher. After try-in with pasters the L＊ and the b＊ value had slightly increased,while the a＊ value decreased a little compared with those of before try-in. The veneered teeth after try-in with White opaque paster exhibited higher L＊ and b＊ value and lower a＊ value compared with other four paster,and was very close to the veneers itself in terms of L＊a＊b＊ values with an average chromatism of 0.43NBS.Conclusion IPS-EMPRESSⅡ ceramic veneers can improve the color of tetracycline teeth,however,the final color of the veneered teeth is easily influenced by the color of the abutment teeth. A luting agent with color masking ability can do some improvements in the final color.
