云南武定县非创伤性充填术治疗儿童恒牙龋坏的临床观察

摘　　要：目的对学龄儿童的恒磨牙龋坏进行ART治疗,评价充填后2a的临床效果,比较乡村医生与实习医生的充填效果,为ART充填技术在云南省农村推广普及的可行性研究提供依据.方法对云南省楚雄州乡村小学的189名儿童、共217颗龋坏恒磨牙恒磨牙龋坏进行非创伤性充填(atraumatic restorative treatment,ART),于充填后6月、12月、18月、24月复查评价,比较乡村医生与实习医生的充填效果,结果95.65%的儿童在治疗中无疼痛,95.74%的儿童愿意再次接受治疗.充填后6个月、12个月、18个月、24个月的失败率分别为8.7%、13.0%、25.7%和28.0%.治疗后6个月、18个月乡村医生与实习医生完成的ART充填体成功率之间的比较有统计学差异(P<0.05),而充填后12月和24月两组之间成功率无统计学差异(P>0.05).结论ART产填术易于操作,2a的观察成功率在60%以上,乡村医生可掌握该技术并可取得较好的效果.Objective To provide restorations for caries in permanent teeth of inschool children with ART in Wuding county, Yunnan province, to evaluate the 24-month restorative results and to compare the restorations results operated by dental students and village doctors. Methods A total of 217 ART restorations were finished in 189 children, by dental students and village doctors respectively. The restorations effects were evaluated every six months within two years. Results The failure rate of ART restorations after 6-month, 12-month, 18-mon- th and 24-month were 8.7%, 13.0%, 25.7% and 28.0% respectively. The cumulative 6-month and 1 8 -month survival rates of restorations finished by dental students were higher than the ones finished by village doctors; however, there was no significant difference in the cumulative 12-month and 24-month survival rate of restorations between finished by village doctors and by dental students respectively. Conclusions The ART technique is easy to master and is highly accepted by patients. The success rate of restorations is over 60% in 2-year follow-up investigation. Village doctors can master the ART procedures and gain good clinical effects.
