摘 要:目的探讨乳腺癌新辅助化疗后DCE-MRI病灶形态学及TIC的变化规律。方法组织学活检证实乳腺癌患者12例在新辅助化疗前、化疗结束后手术前行MRI动态增强扫描,观察肿瘤强化形态及比较TIC的变化。结果12例患者中2例原肿瘤强化区域无强化,残余10例病变最大增强斜率为(236.±382.7),比化疗前的(531.4±79.6)显著减低,曲线类型有由Ⅲ、Ⅱ型向Ⅰ型转变趋势。结论乳腺癌新辅助化疗后残余病变增强速率显著减低。时间信号曲线类型有由流出型向流入型转变的趋势。Objective To investigate the alteration of dynamic enhancement of breast cancer after neoadjuvant chemotherapy (NCT) by dynamic contrast-enhanced MR imaging (DCE-MRI).Methods 12 patients with pathologically confirmed breast carcinoma had DCE-MRI examination before and after NCT.DCE-MRI was performed on a 1.0T MR scanner using STIR (short T1 inversion recovery) before and repeated 6 times after administration of GD-DTPA. The type of time signal intensity curve (TIC), pattern of residual enhancement, maximum slope of each breast cancer before and after chemotherapy were recorded. Results 10 of 12 cases showed residual enhancement, while 2 cases showed no enhancement. Of the 10 cases with residual enhancement, 5 showed non-mass-like enhancement and 5 cases showed mass-like enhancement. The mean value of maximum enhancement slope of the 10 cases with residual enhancement after chemotherapy is lower than its value before chemotherapy. The type of curve before chemotherapy changed from III/ II to II/ I in the cases (7/10) with residual enhancement. Conclusion Residual disease of breast cancer after NCT enhanced at a slow speed and with decreased blood flow. Time signal intensity curve tend to change from type III /II to type I .
