摘 要:目的观察冷冻联合粒巨细胞集落刺激因子(GM—CSF)治疗前列腺癌前后机体抗肿瘤免疫反应,及其对肺转移瘤的影响。方法复制前列腺癌皮下移植瘤。将动物模型随机分为4组:A组(单纯冷冻组)、B组(冷冻联合GM—CSF组)、C组(GM—CSF组)、D组(对照组)。动态观察治疗前、治疗后7、14、21d肿瘤体积、肺转移瘤发生率。分离脾脏淋巴细胞,酶联免疫斑点(ELIS—POT)试验检测分泌肿瘤特异性干扰素(IFN)-γ的Th细胞数。乳酸脱氢酶(LDH)释放试验检测细胞毒性T淋巴细胞(CTL)肿瘤特异性杀伤活性。结果治疗后21d,A组、B组、C组、D组平均肿瘤体积分别是1.22、0.26、2.69、2.70cm^3;肺转移瘤发生率分别是:3/5、0/5、3/5、4/5;每1×10^5个脾细胞中肿瘤特异性IFN-γ^+Th细胞数分别是:35.80±1.48、239.00±1.58、16.40±1.14、14.80±0.83;CTL特异杀伤活性分别为:(8.54±0.11)%、(44.50±0.85)%、(8.04±0.18)%、(6.92±0.41)%。结论冷冻联合GM—CSF可明显增强机体抗肿瘤特异性免疫反应,有效防治肺转移瘤。Objective To assess the anti-tumor immune response after combined cryoablation with GM-CSF treatment for prostate cancer,and the effect on lung metastasis. Methods Mouse model of prostate cancer was used, and the tumor-bearing mice were divided into four groups : group A ( cryoablation treatment) ,group B (combined cryoabaltion and GM-CSF treatment), group C (GM-CSF treatment), group D (control group). Gross tumor volume,and the rate of lung metastasis were assessed on the posttreatment day 0,7,14,21. Lymphocytes of spleen were isolated, and the number of tumor-specific IFN-γ+ Th cells after treatment was counted by the method of enzyme linked immunospot assay (ELISPOT). And tumor-specific cytolytie activity of cytotoxic T lymphocyte (CTL) was measured by LDH assay. Results On the post-treatment day 21, the gross tumor volume in groups A, B, C, and D was 1.22,0.26,2.69, and 2.70 cm^3 ,the rate of lung metastasis was 3/5,0/5,3/5, and 4/5, the number of IFN-γ^+ cells every 1 × 10^5 Th ceils was 35.80 ± 1.48,239.00 ± 1.58,16.40 ± 1.14, and 14.80 ± 0.83,cytolytic activity of CTL against prostate cancer cells was ( 8.54 ± 0.11 ) % , (44.50 ± 0.85 ) % , ( 8.04 ± 0.18 ) %, and ( 6.92 ± 0.41 )% respectively. Conclusion Combined cryoablation with GM-CSF treatment could increase tumorspecific immune response, and prevent lung metastasis effectively.
