摘  要:目的:探讨大鼠创伤性脑损伤(TBI)肝早期细胞凋亡和肿瘤坏死因子-α(TNF-α)的变化及与应激性肝损害(HSI)的关系。方法:用改良Allen法建立TBI模型。40只健康雄性Wister大鼠随机分为5组,正常对照组和TBI后6、12、24、48h时相组。测定各组大鼠血清ALT、AST和TNF-α水平,采用流式细胞术检测肝早期细胞凋亡率,透射电镜观察肝脏早期凋亡细胞,并观察肝组织超微结构的改变。结果:TBI早期,各组血清ALT、AST和TNF-α水平升高,与对照组比较差异显著;早期细胞凋亡率于伤后6h增多并达峰值。TBI后各组光镜和电镜可观察到肝组织不同程度受损。结论:TBI后肝早期细胞凋亡和TNF-α增加可能是继发HSI的重要机制之一。Objective: To explore the changes of hepatocyte apoptosis and tumor necrosis factor-α(TNF-α)at early stage after traumatic brain injury(TBI)in rats and the effect on hepatic stress injury(HSl). Methods: The animal model was built by using an improved Allen' s method.Forty healthy male Wistar rats had been divided randomly into five groups:control group,TBI group which was divided into four subgroups according to different time:6h, 12h,24h and 48h after acute brain trauma.The serum levels of ALT, AST and TNF-α were measured.The hepatocyte apoptosis at early stage was detected through transmission electron microscope and flow cytometer.And pathological changes of liver tissue under electron microscope were observed. Results: At early period of TBI the serum levels of ALT, AST and TNF-α were increased in TBI group,which were significantly different compared with control group.The rate of apoptosis got to a peak at 6h after TBI.Injuries of liver tissue at different degrees were observed in TBI group. Conclusion: Apoptosis at early stage and TNF-α may be involved in the pathogenesis of HSI which occur at early period after TBI.
