自体外周血造血干细胞移植治疗帕金森病10例

摘　　要：2004/2006宜昌市第一人民医院收治的10例单侧受累帕金森病患者,男7例,女3例,平均年龄(64.2±8.2)岁,平均病程(10.2±2.6)年,均符合2006年中华医学会神经病学分会运动障碍及帕金森病组制订的诊断标准。按患者意愿,10例患者分为细胞移植组、对照组,5例/组。对照组患者采用常规复方左旋多巴制剂进行药物治疗,细胞移植组在其基础上给予自体外周血造血干细胞移植,单个核细胞数为(5～8)×108/kg,CD34+细胞数为(4～8)×106/kg。与移植前比较,移植后1个月细胞移植组无明显不良反应,3例部分缓解,在UPDRS评分运动部分、运动迟缓、僵直、日常生活能力方面有明显改善(P<0.05),2例未缓解;对照组2例部分缓解,3例未缓解。两组间除僵直指标无差异外,余指标比较均有明显差异(P<0.05)。提示自体外周血造血干细胞移植治疗帕金森病对部分患者是安全有效的。A total of 10 unilateral involved Parkinson＇s disease patients, 7 males and 3 females, with an average age of （64.2±8.2） years, mean course of disease of （10.2±2.6） years, were recruited at the Yichang First People＇s Hospital from 2004 to 2006. The subjects were in accordance with diagnostic criteria of dyskinesia and Parkinson＇s disease, formulated by Chinese Medical Association, Neurology Branch in 2006. Ten patients were equally divided into cell transplantation and control groups according to patients＇ will. Patients in the control group were treated with compound levodopa preparation. Patients in the cell transplantation group underwent autologous peripheral blood hematopoietic stem cells, except for above-mentioned treatment, with mononuclear cells of （5-8） ×10^8/kg, and CD34＋ cells of （4-8） ×10^8/kg. No significant adverse side effect was determined in the cell transplantation group 1 month following transplantation, and partial remission was found in 3 cases. UPDRS, bradykinesia, rigidity, and activity of daily living were significantly improved （P 〈 0.05）, and no remission was seen in 2 cases. Partial remission was found in 2 cases, and no remission was detected in 3 cases in the control group. No significant difference was determined in rigidity between both groups, and significant differences in other indices were found （P 〈 0.05）. It is suggested that it is safe and effective to use autologous peripheral blood hematopoietic stem cell transplantation for treating Parkinson＇s disease in some patients.
