摘 要:目的:研究18F-氟脱氧葡萄糖(18F-fluorodeoxyglucose,18F-FDG)双探头符合线路单光子反射计算机断层成像(single-photon emission computed tomography,SPECT)技术在肺部肿瘤性质诊断和鉴别诊断中的应用价值。方法:对80例肺部肿瘤患者进行18F-FDG双探头符合线路SPECT显像和CT检查,并与病理结果进行对照,符合线路SPECT仪的资料采用目测法及半定量法(L/B值)进行分析。结果:目测法18F-FDG双探头符合线路SPECT诊断肺部肿瘤性质的敏感度和准确度分别是96.8%和87.5%,而CT诊断肺部肿瘤性质的敏感度和准确度分别为74.2%和67.5%,2种方法比较差异有统计学意义(P<0.05)。半定量法18F-FDG双探头符合线路SPECT仪诊断肺部肿瘤性质的敏感度和准确度分别是93.5%和87.5%,与CT相比差异有统计学意义(P<0.05)。肺部恶性肿瘤的L/B值为9.5±5.1,良性病变的L/B值为3.7±4.2,2者差异有统计学意义(P<0.05)。18F-FDG双探头符合线路SPECT和CT这2种检查方法在术前对纵隔淋巴结转移诊断的敏感度及符合率(分别为66.7%和63.6%以及55.6%和54.5%)与病理检查结果比较,2者差异无统计学意义。结论:与CT相比,18F-FDG双探头符合线路SPECT能更准确地鉴别肺部肿瘤性质,是一种较好的无创性肺癌诊断技术。Objective:To study the value of 18F-FDG dual head SPECT coincidence imaging in the identification and differential diagnosis of lung cancer.Methods:Eighty patients with lung cancer were examined with 18F-FDG dual head SPECT coincidence imaging and CT scan,respectively.The imaging results were compared with that of pathological examination.Dual head SPECT data were semi-quantitively analyzed by calculating ratio of lesion to background counts(L/B method) and visual method.Results of dual head SPECT and CT were compared with those of pathological examination.Results:When the visual method was used,the sensitivity and accuracy of 18F-FDG dual head SPECT coincidence imaging for detecting malignant mass were 96.8% and 87.5%,respectively.Whereas the sensitivity and accuracy of CT scan were 74.2% and 67.5%,respectively.The difference was significant(P〈0.05).When semi-quantitive method was used,the sensitivity and accuracy for detection of malignant mass of dual head SPECT were 93.5% and 87.5%,respectively,which were significantly different from those of CT scan(P〈0.05).The L/B value was 9.5±5.1 for pulmonary malignant mass and 3.7±4.2 for benign pulmonary disease.The difference was significant(P〈0.05).Compared with the results by pathological examination,the sensitivity and accordance rate of dual head SPECT and CT scan for diagnosis of preoperative mediastinal lymph node metastasis was 66.7% vs 63.6% and 55.6% vs 54.5%,and the difference was not significant(P〉0.05).Conclusion:Compared with CT scan,18F-FDG dual head SPECT coincidence imaging discriminates the features of pulmonary tumors more accurately.Dual head SPECT coincidence imaging is a better injury-free diagnosis method.
