老年人急性上消化道大量出血的临床研究

摘　　要：目的探讨老年人急性上消化道大量出血的临床特点及治疗。方法对本院2005年1月至2007年12月收治的老年人急性上消化道大量出血病例资料进行分析。结果58例老年人上消化道大出血患者均有出血前用药史，呕血53例，仅黑便5例，消化性溃疡43例，急性糜烂性胃炎5例，胃癌3例，食管癌2例，食管胃底静脉曲张2例，其他3例。仅1例手术后并发肺栓塞死亡，其余均内科综合治疗出血停止后出院。结论老年人急性上消化道大量出血与非甾体抗炎药有关，以消化性溃疡多见，急性胃黏膜损害及肿瘤亦占相当比例，内科综合治疗效果好，积极预防有望减少出血。Objective To explore aged patients with acute upper gastrointestinal massive hemorrhage Clinical features and treatment. Methods From January 2005 to December 2007, the clinical data of aged patients with acute upper gastrointestinal massive hemorrhage in our hospital were analysed. Results 58 cases of aged patients with acute upper gastrointestinal massive hemorrhage were bleeding premedication history,53 cases of hematemesis, melena only 5 cases ,43 cases of peptic ulcer, acute erosive gastritis 5 cases, 3 cases of gastric cancer,2 cases of esophageal cancer, esophageal 2 cases of gastric varices, and other three cases. Only one cases of pulmonary embolism after surgery complicated by the death, and the rest are internal medicine combined ther- apy to stop bleeding and discharged. Conclusion Aged patients with acute upper gastrointestinal massive hem- orrhage with non-steroidal anti-inflammatory drug related to peptic ulcer often, acute gastric mucosal damage and also accounted for a considerable proportion of the tumor, internal medicine comprehensive treatment, and prevention is expected to reduce bleeding.
