摘 要:目的建立北京地区儿童全血19种氨基酸含量的正常参考值范围,为临床诊治氨基酸代谢异常性疾病提供依据。方法采用串联质谱法检测北京地区1050倒正常儿童的19种氨基酸水平,其中包括有赖氨酸(Lys),苯丙氨酸(Phe),蛋氨酸(Met),苏氨酸(Thr),异亮-亮氨酸(Leu—Ile),缬氨酸(Val),丙氨酸(Ala),精氨酸(Arg),天冬氨酸(Asp),瓜氨酸(Cit),半胱氨酸(Cys),谷氨酸(Glu),谷氨酰胺(Gln),甘氨酸(Gly),组氨酸(His),鸟氨酸(Orn),脯氨酸(Pro),丝氨酸(Ser),酪氨酸(Tyr)。将所有儿童根据性别不同分成男、女两组;根据发育情况分为婴儿组、幼儿组、学龄前组、学龄组、青春发育期组。分析比较不同性剐和不同年龄组之间的氨基酸水平是否存在差异,进而得到串联质谱检测儿童干血滤纸片氨基酸参考范围。结果经过正态性检验,儿童氨基酸水平成偏态分布,因此,所有数据均经过对数转换后进行统计分析。经统计后发现不同性别儿童的氨基酸水平之间并无显著性差异(P〉0.05)。不同年龄段儿童的部分氨基酸水平(Ala,Asp,Gln,Glu,Ser,Thr,Pro)存在一定差异(P〈0.05)。根据年龄不同,计算各种氨基酸的正常参考范围。结论根据年龄的不同,建立串联质谱法检测儿童的各种氨基酸的正常参考范围对于临床诊治和筛查氨基酸代谢异常性疾病十分重要。Objective To build the correct reference values of amino acids of children in Beijing and provide clinicians with laboratory evidence. Methods The levels of 19 amino acids on the dry blood spots of 1 050 healthy children in Beijing were detected by tandem mass spectrometry. 19 amino acids included Lys, Phe, Met, Thr, Leu-Ile, Val, Ala, Arg, Asp, Cit, Cys, Glu ,Gln,Gly,His,Orn,Pro,Ser and Tyr. According to different genders,children were divided into male and female two groups;according to developmental conditions,the children were divided into infancy group,toddler's age group ,preschool age group,school age group ,adolescence group. The levels of amino acids of different gender groups and age groups were compared ,and the reference values of amino acids on the dry blood spots of children were calculated. Results According to the results of test of normality the levels of amino acids of children belong to skewness distribution. Transformations of normality were done by all data of amino acids. There were not obvious significance of amino acids in different gender groups (P〉0. 05). There were obvious significance of amino acids in different age groups (P〈0. 05). According to different ages ,the reference values of amino acids of children were calculated. Conclusions It is a very important job to provide clinicians with correct reference values of amino acids of children detected by tandem mass spectrometry.
