摘 要:目的:分析低场MRI上泌乳素瘤的表现与血清PRL水平之间的关系。材料与方法:回顾性分析42例经我院内分泌科及部分病例经手术病理证实为泌乳素瘤病例的低场MRI表现并与其术前血清PRL水平进行对比。结果:泌乳素瘤42例,位于垂体侧翼28例,位于中间部6例,类圆形或椭圆形10例,分叶状或不规则形26例。低场MRI平扫T1WI上26例呈低信号,等信号13例,略高信号3例。合并坏死囊变部分在T1WI上呈低信号,合并出血灶者出血灶在T1WI上呈高信号;注射GD-DTPA后即扫描,T1WI上25例呈相对低信号,延迟扫描呈相对高信号,5例呈相对高信号,延迟扫描呈等信号。本组病例肿瘤合并出血、坏死囊变比例不高,约占16.67%(7/42)。泌乳素瘤平均直径为0.6650±0.14707mm,术前血清PRL平均值为62.25±1.78ng/ml,瘤体的大小与血清PRL水平无直线相关关系(r=0.004,P=0.979)。结论:垂体泌乳素瘤的大小与血清PRL水平无直线相关关系。Purpose: To assess the relationship between the Low Field MRI appearances of pituitary prolactionoma and serum PRL level. Materials and Methods: The low Field MRI characteristic appearances of 42 cases with surgically and pathologically proved pituitary porlactionoma were retrospectively analyzed, and compared with the preoperative serum PRL level. Results: 28 of 42 cases were located in the lateral wings of pituitary gland, 6 cases were in the central region, 10 cases were round or oval in shape, 26 cases were lobular - or irregular - shaped. On unenhanced MILl, 26 cases showed hypointensity, 13 cases showed isointensity and 3 cases showed slightly hyperintensity, on T1WI while cases with necrosis and cystis degeneration showed hypointensity, cases with hemorrhage showed hyperintensity; On GD - DTPA T1 - weighted images, On T1WI , 25 cases showed relatively hypointensity, delayed scanning showed relative hyperintensity; 5 cases showed hyperintensity, delayed scanning showed relatively isointensity. The percentage of hemorrhage, necrosis and cystis degeneration is 16. 67% (7/42). The average diameter of prolactionoma is 0. 6650 ±0. 1471mm, and its mean level of serum PRL is 62. 25 ± 1.78ng/ml. There is no positive linear correlation between the size of pituitary prolactionoma and serum PRL level( r = 0. 004, P = 0. 979). Conclusion: The correlation between gross prolactionoma volume and serum PRL level is avalilable, but it is not the positive coUinearity.
