MRI在脑胶质肉瘤诊断中的应用

摘　　要：目的分析脑胶质肉瘤的MRI影像学特征。方法回顾性分析6例经手术及病理证实的胶质肉瘤的MRI表现及临床诊疗过程，并结合文献分析其临床影像学特征。结果6例胶质肉瘤均位于左侧幕上半球。在MRIT1WI上，胶质肉瘤呈不规则低信号3例，等低信号3例，T2WI呈不均匀高信号4例，稍高信号2例，可见囊变坏死及周围明显水肿，增强后4例呈周边明显环形或结节样强化，1例呈不均匀轻中度强化，1例呈明显均匀强化。瘤周水肿重度4例，中度1例，无水肿1例。结论脑胶质肉瘤大多具有较典型的MRI特征，少数表现亦可多样。Objective To analyze MRI features of gliosarcomas of brain. Methods MR features of cerebral gliosarcomas confirmed by surgery and pathology in 6 cases were retrospectively analyzed with review of literatures. Results The lesions in 6 patients were all located at the left and supratentorial region. The lesions were hypointensity （3 cases） or isointensity to slightly hypointensity （3 cases） on T1WI,and hyperintensity（4 cases） or slightly hyperintensity（2 cases） on T2WI. After Gd-DTPA injection,obviously ring- shaped or nodule- like enhancement in 4, obviously homogeneous enhancement in one and moderate unhomogeneous ＂enhancement in one were showed. There was marked peritumorous edema in 4 patients,moderate edema in 1 patient and no edema in 1 patient. Conclusion Most of cerebral gliosarcomas has typical MRI features,but various features are also demonstrated in some cases.
