血清胃蛋白酶原在胃癌治疗过程中的变化及其意义

摘　　要：目的探讨血清胃蛋白酶原PGⅠ、PGⅡ在胃癌治疗过程中的变化及其临床意义。方法用ELISA法检测126例胃癌患者手术前后和41例复发患者以及30例健康体检者(对照组)血清PGⅠ、PGⅡ含量,并计算PGR(PGⅠ/PGⅡ)值。结果胃癌组手术前血清胃蛋白酶原PGⅠ、PGⅡ含量与对照组存在差异(P<0.05),在胃癌不同病理分期中没有差异。手术后PGⅠ、PGⅡ浓度与手术前相比在全胃切除组差异有统计学意义(P<0.01),在部分胃切除组差异有统计学意义(P<0.05)。复发、转移者中血清PGⅠ、PGⅡ含量与手术后相比在全胃切除组差异有统计学意义(P<0.05),在部分胃切除组差异无统计学意义。结论血清PGⅠ、PGⅡ水平与胃癌病理分期无关,与诊断、复发有关,监测其变化可作为胃癌全胃切除术后随诊的一项血清学指标。Objective To study serum pepsinogen I（PGI）,serum pepsinogen II（PGII）level in patients with gastric cancer before and after treatment.Methods Serum PGI and PGII levels were detected by enzyme linked immunosorbent assay（ELISA） in 30 cases of healthy controls,41 patients with recurrence gastric cancer,and 126 patients with gastric cancer who underwent gastrectomy.Results Before operation,the serum levels of PGI and PGII were significantly different between gastric cancer and healthy controls,but there was no difference in patients with different pathological phases.Compared with pre-operation,serum levels of PGI and PGR were different in patients with gastric cancer who underwent either porximal gastrectomy or totatl gastrectomy.Conclusion The determination of serum PGI and PGII was not helpful for evaluating pathological phases of gastric cancer,but can provide important clues in detecting recurrence of gastric cancer.
