萎缩性胃炎表皮生长因子及其受体表达的临床意义探讨

摘　　要：目的通过对萎缩性胃炎表皮生长因子（EGF）和表皮生长因子受体（EGFR测定，探讨EGFR对萎缩性胃炎转归影响作用。方法应用EGFR单抗免疫组化ABC技术和放射免疫法对50例萎缩性胃炎和25例胃癌及19例“正常”胃粘膜组织EGFR和血清EGF水平进行检测。结果萎缩性胃炎EGFR表达的阳性率为60％，血清EGF水平平均3．54土1．47ug／L，较“正常”组（11％和1．77土0．60μg／L）明显增高，与胃癌（73％，3．72士1．84μg／L）十分接近。伴有Ⅱb型肠化者EGF水平更高。并发现EGF的高水平与EGFR高表达具有一致性。结论萎缩性胃炎体内有高水平EGF和EGFR表达。对目前使用促EGF药物治疗萎缩性胃炎提出质疑。Objective To study the effect of EGF in atrophic gastritis. Methods This study measured the level of EGFR in gastric mucosa and serum EGF in 50 cases of atrophic gastritis, 25 gastric cancer and 19 normal gastric mucosa respectively by ABC technique and RIA. Results The positive rate ofEGFR expression in atrophic gastritis was 60% and average level of serum level EGF was 3.54±1.47μg/L. Both were obviously higher than those in normal group (11 %, 1.77t 0.60μg/L) and were similiar to those seenin gastric cancer group(73%, 3.72± 1.84μg/L), level of serum EGF was higherin patients with type Ⅱ b intestine metaplasia and which was consistent with high EGFR expression. Conclusion This shows there are a high levels of serum EGF and EGFR expression in atrophic gastritis, and it becomes doubtful that the currently used EGF promoter is really beneficial in the treatment of atrophic gastritis.
