中医情志护理对慢性阻塞性肺病患者焦虑心理及上机影响的临床观察

摘　　要：目的:观察应用情志护理对慢性阻塞性肺病(COPD)患者使用呼吸机期间焦虑心理及其对上机时间的影响探讨并建立一套对COPD患者上机期间焦虑心理的护理措施。方法:选取88例初次使用呼吸机意识清楚的COPD患者,随机分为对照组和观察组,每组44例,对照组予以常规的治疗护理,观察组在此基础上运用情志护理。比较分析两组患者初上机时(干预前)与上机后72h(干预后)的焦虑心理及治疗期间的平均上机时间。结果:两组患者干预前中度以上焦虑的发生率分别为16例(占36.36%)和17例(占38.64%),差异无统计学意义(P>0.05)干预后中度以上焦虑的发生率分别为12例(占27.27%)和3例(占6.82%),差异有统计学意义(P<0.05),观察组与组内干预前比较差异有统计学意义(P<0.01);两组患者治疗期间平均上机时间分别为(14.11±8.01)天和(10.16±7.69)天,差异有统计学意义(P<0.05)。结论:运用情志护理进行心理护理干预能有效缓解患者的焦虑情绪,缩短患者呼吸机的使用时间,有助于疾病的康复,提高患者生活质量,减少医疗费用,节约医疗资源。Objective ：To observe psychological anxiety and remaining on the ventilator in patients with chronic obstructive pulmonary disease by emotion nursery of traditional Chinese medicine, to discuss and establish a nursing measure based on anxiety neurosis. Methodz：88 patients with COPD were randomly divided into trial group and control group, and each group was 44 patients. They were randomized to routine nursing alone or in combination with emotion nursery of traditional Chinese medicine. Anxiety neurosis at the starting time and after 7t hours and mean time to using veptilators during treatment in two groups patients were analyzed and compared. Results ：The incidence with anxiety neurosis in moderate or above were 0.77% and 0.75% respectively and was similar between the two groups before intervention （ P 〉 0.05 ） ; They were 0.77% and 0.75% respectively after intervention （ P 〈 0.05 ）, it is the difference between before and after the intervention （ P 〈 0.05） ; The median times remaining on the ventilator were 14.11 ± 8.01 and 10.16 ± 7.69 days respectively during treatment in the two groups （ P 〈 0.05 ）. Conclusion ： Psychological nursing intervention of using emotion nursery of traditional Chinese medicine can be effective in relieving psychological anxiety, reduce the time of using ventilators, be helpful for the recovery of patient, improve quality of life, save medical expense and medical resources.
