摘  要:目的:探讨子宫颈癌患者hprt基因突变率、微核率和姐妹染色单体交换SCE率的自发水平及放疗后变化,评估电离辐射诱发的染色体损伤。方法:取25例宫颈鳞癌患者放疗前与放疗中外周静脉血,多核细胞法测hprt突变率,胞质分裂阻滞法测微核率,姐妹染色单体交换试验测姐妹染色单体交换率。正常对照组共25例,来自健康查体者。结果:(1)子宫颈癌患者自发hprt基因突变率、微核率、姐妹染色单体交换率均高于健康人(P均﹤0.05)。(2)hprt突变率在放疗后升高,40Gy时达最高峰,40Gy后稍下降,差别有显著性(P<0.05),呈剂量-反应关系(r=0.620,P<0.01)。(3)微核率在放疗后逐渐升高并呈线性关系(r=0.827,P<0.01)。(4)各累积剂量组SCE率均高于治疗前,差异有统计学意义(P均﹤0.05),但无明显的线性关系。结论:在一定的放射剂量范围内,外周血淋巴细胞hprt突变率、微核率与姐妹染色单体交换率是评估放射损伤的有效生物指标。Objective: To study the change of the frequency of hypoxanthine-guanine phosphoribosyl transferase (hprt) gene mutation, micronucleus (MN) and sister ehromatid exchange (SCE) in patients with cervical carcinoma;and evaluate the chromosome damage induced by radiation. Methods: 25 peripheral blood samples of patieuts with cervical carcinoma were collected before and during radiotherapy. The hprt gene mutation was studied by muhinucleated cell assay, cytokinesis-blocked MN assay and the SCE assay were applied to assess the genetic damage. 25 cases from healthy persons were in the normal control group. Results: ( 1 ) Significant difference of mutation frequency of hprt gene, MN frequency and SCE frequency and were observed in cervical carcinoma patients. (2) Mutation frequency of hprt gene increased after radiation and up to the peak at 40Gy , and decreased slightly after that. The linear regression analysis showed a linear dose-effect manner (r=0.620, P〈0.01). (3) The MN frequency increased after radiation and had a linear relationship with the radiation dose accumulated (r=0.827,P〈0.01).(4) After radiotherapy, the average SCE frequency significantly increased but no correlation was found between the SCE frequency and radiation dose. Conclusion: In a certain dose of radiation, the peripheral blood lymphocytes hprt mutation frequency,the MN frequency and the SCE frequency are biological indicators for assessment of the radiation damage.
