摘  要:[目的]探讨碘摄入对新生儿先天性甲状腺功能减退症(CH)发病率的影响。[方法]人群碘营养状况按国家卫生部《IDD防治监测方案(试行)》监测,新生儿筛查按卫生部《新生儿疾病筛查技术规范》进行,以促甲状腺素(TSH)作为筛查指标,实验方法为时间分辨免疫荧光法。[结果]儿童尿碘中位数(MUI)、由实施USI前的(1995年)71.25μg/L(碘缺乏)上升到2005年的170.7μg/L(碘适量),但分析2005年监测儿童中﹤100μg/L占24.9%、﹥300μg/L占18.1%、即有43.0%儿童碘营养仍处于不足或过量;CH发病率2006年36.60/10万(0.036×10-2)、2000~2005年的发病率39.97/10万(0.039×10-2),差异无统计学意义(P﹥0.05),2007年发病率上升至69.75/10万(0.069×10-2),与2006年的发病率比较,差异有统计学意义(P﹤0.01)。[结论]人群碘缺乏或碘过量可能系近年CH发病率上升的主要因素之一。[ Objective] To explore the effect of iodine intake on incidence of congenital hypothyroidism (CH). [Meth- ods] The iodine nutrition condition was monitored according the "National IDD prevention and surveillance". Serum thyroid stimulation hormone (TSH) levels was regarded as the screening index, and thescrecning for neonate CH was detected by time-resolved fluorometry assay (TRFIA) according to the screening for neonate diseases technical regulations. [ResUltS] The median urinary iodine went up from 71.25μg/L in 1995 (iodine deficiency) to 170.7μg/L in 2005 (iodine adequate). The rate of less than 〈 100μg/L was 24.9% and more than 〉 300 μg/L was 18.1% in 2005, that means 43% of children were iodine nutrition deficieng or exceptional. The CH incidence was 36.60/105 in 2006, and was 39.97/105 from 2000 to 2005, and no significant difference was found between the two time periods (P〉 0.05). The CH incidence in 2007 was 69.75/10s, and it showed significant difference compared with that in 2006 (P 〈 0.01). [Conclusion] The abnormal (deficiency or encess) iodine nutrition might be one of the primory reasons for incidence of CH increasing.
