P53、P-gp蛋白在卵巢癌组织中的表达及意义

摘　　要：目的探讨P53、P-糖蛋白(P-gp)在卵巢癌组织中的表达与化疗疗效及预后的关系。方法采用SP免疫组化法检测53例卵巢癌组织石蜡标本P53、P-gp的表达情况,并分析其与卵巢癌患者化疗疗效及预后的关系。53例卵巢癌患者中Ⅰ期6例、Ⅱ期14例、Ⅲ期23例、Ⅳ期10例。结果Ⅰ-Ⅱ期组P53、P-gp的阳性率(35.0%、40.0%)分别低于Ⅲ-Ⅳ组P53、P-gp的阳性率(72.7%、78.8%)(P<0.05)。P53、P-gp阳性表达的患者化疗的有效率分别为35.5%和35.3%,而阴性患者化疗的有效率分别为81.8%、89.5%(P<0.05)。P53、P-gp的阴性、阳性表达在Ⅲ-Ⅳ患者的2年及3年生存率之间差异均有显著性(P<0.05)。结论P53、P-gp的表达与卵巢癌组织化疗耐药及预后存在相关性,P53、P-gp蛋白表达可作为卵巢癌化疗耐药和预后的检测指标。Objective To study the expression of P53, P - gp in ovarian cancer tissue and its relationship with effect of chemotherapy and prognosis. Methods The expression of P53, P - gp was detected by SP immunohistochemieal method in the paraffin specimen of ovarian cancer tissue（53 eases）. Among the 53 ovarian cancer eases, 6 eases are in stage I , 14 eases in stage Ⅱ, 23 eases in stage Ⅲ and 10 cases in stage Ⅳ. Results The positive rates of P53 and P-gp in group of stage I-Ⅱ（35.0% and 40.0% respectively） are lower than those in group of stage Ⅲ-Ⅳ（72.7% and 78.8% respectively）（P〈0.05）. The effective rates of chemotherapy reach 35.5 % and 35.3% respectively in patients with positive expression in P53 and P-gp and 81.8% and 89.5% respectively in patients with negative expression （P〈0.05）. The positive expression in P53 and P - gp differs remarkably from their negative expression in the survival rate after 2 and 3 years among the patients in group Ⅲ-Ⅳ stage（P〈0.05）. Conclusions The expression of P53 and P - gp is related to chemotherapy resistance and prognosis of oval cancer tissue and the protein expression of P53 and P - gp could serve as the detection index of chemotherapy resistance and prognosis of oval cancer.
