替普瑞酮佐治幽门螺杆菌相关性胃溃疡疗效观察

摘　　要：目的探讨替普瑞酮对治疗幽门螺杆菌(HP)相关性胃溃疡愈合质量的影响。方法将125例经胃镜通过快速尿素酶试验及病理学检查(Warrin-Stary染色),2项诊断均为HP阳性感染的胃溃疡患者随机分为对照组(59例)和治疗组(66例),治疗组加服替普瑞酮,余治疗相同。服药1周内记录临床症状消失情况,治疗结束4周后复查胃镜和检测HP,6个月后随访溃疡复发情况。结果治疗组和对照组HP根除率分别为92.42%和91.53%(P>0.05);治疗组腹痛、嗳气、反酸、恶心、腹胀等症状的消失率明显高于对照组(P<0.05);治疗组治疗胃溃疡总有效率(92.42%)明显高于对照组(76.27%)(P<0.05);6个月后治疗组胃溃疡复发率(9.26%)明显低于对照组(25.71%)(P<0.05)。结论替普瑞酮在联合雷贝拉唑和阿莫西林、克拉霉素治疗HP相关性胃溃疡中,具有用药次数少、药物剂量小、不良反应轻,主要症状消失快等优点,可促进溃疡愈合,提高治愈率,降低复发率。Objective To explore the effect of treating HP - related digestive ulcer with teprenone. Methods 125 cases of Hp - related digestive ulcer confirmed by endoscopy and pathology were randomly divided into two groups ： control group and therapy group. 59 cases of control group was administrated with omeprazole （0.5g,Tid）, clarithromycin （0.5g, Bid） and rabeprazole（20 mg,Qd） for two weeks by oral. 66 cases of therapy group was administrated with teprenone（50 mg,tid） besides of omeprazole, clarithromycin and rabeprazole for two weeks by oral. After a week,we observed the primary symptomatic relief of peptic ulcer. Four weeks after finishing medication, endoscopy and HP test were reperformed. After six weeks we observed if the ulcer was recurred. Results The eradication rate of Hp in both control group and therapy group were 92.42% and 91.53% ,respectively. Significant difference was not observed between the two groups （ P 〉 0.05 ）. The symptomatic relief rate of abdominal pain, belching, sour regurgitation, nausea, abdominal distention had significant difference （P〈0.05）. Total efficiency recovery rate of ulcer were 92.42% and 76.27% （ P 〈 0.05）, respectively. The recurrence rate of ulcer was 9.26% and 25.71% （ P 〈 0.05） respectively six weeks after finishing medication. Conclusions Since the small dosage, gently adverse reaction of fraltadone and rapid symptomatic relief of peptic ulcer in therapy group, teprenone combined with rabeprazole, amoxicillin, clarithromycin were able to improve the rate of ulcer healing and reduce the recurrence rate of ulcer.
