摘  要:目的探讨辛伐他汀对急性脑梗死(ACI)患者血清中可溶性细胞间黏附因子-1(ICAM-1)、P-选择素血管细胞间黏附分子-1(VCAM-1)和血脂水平的影响。方法将110例ACI患者随机分为辛伐他汀联合阿司匹林治疗组56例和阿司匹林治疗组54例。分别在发病72h内和治疗后3个月检测血清中ICAM-1、P-选择素和血脂浓度,选50例健康人设为健康对照组。结果辛伐他汀联合组和阿司匹林组的患者发病72h内血清ICAM-1、P-选择素、血CHO和LDL的浓度高于健康对照组(均P<0.05)。与治疗前及阿司匹林组相比,辛伐他汀联合组在治疗第3个月时血清ICAM-1、P-选择素、血CHO、血LDL浓度明显下降(均P<0.05)。结论辛伐他汀有利于降低血清ICAM-1的浓度,调节免疫功能,抑制炎性反应,从而进一步稳定动脉粥样斑块,可作为治疗ACI患者的联合药物。Objective To investigate the effects of simvastatin on serum intercellular adhesion molecule-1 (sICAM 1), p-selectin, and lipid levels in patients with acute cerebral infarction(ACI). Methods 110 cases with ACI were randomly divided into two therapy groups:(1) 56 cases were given simvastatin combined with Aspirin;(2)54 cases were treated with Aspirin only. serum levels of sICAM-1 ,p-selectin and lipid were detected with in 72h in the onset of the disease and 3 months following the treatment. Meanwhile, the same were detected in the control group(50 healthy people). Results Within 72 h, the serum levels of sICAM-1, p-selectin,cholesterol and LDL were higher in patients with ACI than those in control group(both P〈0. 05). Compared with that before treatment and Aspirin therapy group, the serum levels of slCAM-1, p-selectin, cholesterol and LDL were obviously decreased in Pravastatin combined with Aspirintherapy group 3 months after the treatment(both P〈0.05). Conclusion Simvastatin may benefit lowering the serum levels of sICAM-1 and p selectin,modulating immunological function and restraining inflammation so that it can further stabilize atheromatous plaque. Therefore,Pravastatin combined with Aspirin can be a good approach to treating ACI.
