摘 要:[目的]分析双表型急性白血病临床生物学特征。[方法]对22例BAL患者的骨髓标本分别进行光镜细胞形态学及相关细胞化学染色观察以确定其FAB分型,应用流式细胞仪做免疫分型检测一系列相关单抗,采用兼顾AML和ALL的治疗方案。[结果]BAL的FAB分型以AML-M1/M2,ALL为主;免疫分型以B系、髓系混合表达多见,CD34在BAL中呈高表达并且是对患者预后影响力最大的因素(P=0.03);患者对治疗反应差、缓解率低。[结论]BAL属特殊类型白血病,有独特的临床生物学特征,对化疗方案不敏感,预后较差。[Objective] To analyze the clinical and laboratorial features of biphenotypic acute leukemia (BAL). [Methods] 22 patients with BAL were diagnosed according to the results of morphology, cytochemistry and immunophenotyping of bone marrow cells. The therapeutic protocol was made for both AML and ALL. [ Results ] Of the 22 patients, the main FAB subtypes were AML- M1/M2 and ALL. B-Ly+/My+ phenotype was more frequent. The rate of CD34 antigen expression was high in BAL And it had strong effect on prognosis (P = 0.03). 22 patients got the low complete remission rate and bad effect of therapy. [Conclusion] BAL belongs to one kind of specific leukemia, which has the distinct characteristics. The patients with BAL have bad therapeutic effect and prognosis.
