摘 要:目的:检测OPN和CD44在梗阻性肾病大鼠中的表达,探讨其在肾小管间质纤维化(TIF)病变过程中的作用。方法:利用SD大鼠行单侧输尿管梗阻(UUO)诱导建立TIF动物模型,48只SD大鼠随机分为假手术组(24只)和模型组(24只)。两组分别于实验第7、14、21天三个时间点收获动物各8只。检测上述时间点各组动物肾小管损伤、间质炎症细胞浸润和纤维化等肾脏组织病理学变化特点,采用免疫组化法观察OPN、CD44在梗阻性肾病大鼠中的表达以及桂枝茯苓胶囊对OPN和CD44表达的影响。结果:梗阻性肾病大鼠在上述三个时间点肾小管间质损伤呈进行性加重。免疫组化结果显示,OPN和CD44在TIF中呈高表达,且随着TIF的进展不断增强。OPN和CD44在TIF中的表达同小管间质损伤程度呈正相关(r=0.782,P<0.01;r=0.774,P<0.01)。结论:梗阻性肾病局部OPN和CD44表达明显升高。OPN和CD44在肾损伤中的表达上调,并介导肾间质巨噬细胞浸润和间质纤维化的发展,二者相互作用参与肾脏的发病过程。Objective: To determine the expression of OPN and CD44 in rat kidney with obstructive nephropathy, and in- vestigate the potential role of OPN and CD44 in the pathogenesis of the tubulointerstitial fibrosis (TIF) nephropathy in rats. Methods: Tubulointerstitial fibrosis rat model was established by unilateral ureteral obstruction (UUO). 48 Sprague-Dawley male rats were randomly divided into two groups: model group (n=24) and sham-operated group (n=24). 8 rats were obtained at day 7, 14 and 21 of the experiment. Histological changes were observed in tubulointerstitium injury under microscope. Immunohistochemical method was performed to investigate the expression of OPN and CDJA in TIF and the effect of Guizhifuling Capsules on the expression of OPN and CD44. Results: Renal tubulointerstitium injury aggravated increasing- ly at the three time points. Immunohistochemistry results showed that OPN and CD44 presented high expression in TIF, and they expressed increasingly with the development of TIF. The expression of OPN and CD44 in TIF presented positive correlation with the degree of renal tubulointerstitium injury (r=0.78~, P〈0.01; r=0.774, P〈0.01). Conclusion: The expres- sion of OPN and CD44 obviously increases in the locality of obstructive nephropathy. The expression of OPN and CD44 is increased in the tubulointerstitium injury and mediates the mononuclear cell infiltration as well as the development of TIF.
