摘　　要：目的：评价(99m)锝-甲氧基异丁基异腈（(99)TCm－MIBI）核素闪烁显像对诊断及鉴别诊断乳腺良、恶性包块和腋下淋巴结转移癌的临床价值．方法：对67例临床拟诊的乳腺包块患者进行了99TCm－MIBI乳腺核素前位及侧位闪烁显像（SMM）诊断．全部影像结果均与病理组织检查对照．结果：99TCm－MIBI显像诊断与鉴别诊断乳腺良、恶性肿物的灵敏度、特异性和总诊断符合率分别为92．0％，85．7％及88．1％．对25例病理学证实为乳腺癌患者腋下淋巴结转移瘤的诊断灵敏度、特异性和正确诊断率分别是71．4％，90．9％及80．0％．结论：99TCm－MIBI核素乳腺显像对诊断及鉴别诊断乳腺良、恶性包块及腋下淋巴结转移癌具有较好的临床应用价值，是一个灵敏、特异、安全、无创的好方法．Aim: To evaluate the clinical value of 99TcmMIBI imaging in differentiating the benign, malignantbreast tumor and axillary lymph node metastases [LNM] inpatients prior to biopsy. Methods: 67 patients who received740 MBq 99Tcm-MIBI intravenously were studied. Lateraland anterior planar images were acquired within 15-30 minof the injection. All the results were compared withpathologic study. Results: We report an overall sensitivity of92. 0%, specificity of 85. 7% and crude agreement of88.1% for the diagnosis of breast cancer. To identifyaxillary LNM in patients with breast cancer, the diagnostic sen-sitivity, spcificity and accuracy were 71. 4%, 90. 9% and80. 0% respectively. Conclusion: Mammoscintigraphy with99Tcm-MIBI has high sensitivity and adequate specificity forthe noninvasive differential diagnosis of breast tumor. It isalso useful for diagnosis axillary LNM of breast cancer.
