摘  要:目的研究博莱霉素致大鼠肺纤维化过程中Ⅰ、Ⅲ、Ⅳ和Ⅴ型胶原在肺组织中的异常沉积和分布规律。方法气管内滴入博莱霉素建立大鼠肺纤维化模型。免疫组织化学染色显示Ⅰ、Ⅲ、Ⅳ和Ⅴ型胶原表达,并应用图像分析方法进行相对定量。结果Ⅰ、Ⅲ、Ⅴ型胶原在肺间质中重叠分布,随着纤维化的进展,其相对含量不断增加。Ⅰ型胶原增加最突出,Ⅲ型胶原也呈现逐渐增加态势,Ⅴ型胶原虽然相对含量较低,但也呈现缓慢增多趋势。分布于肺泡上皮细胞和毛细血管内皮细胞基底膜的Ⅳ型胶原在给药3 d出现明显的断裂、缺失,28 d时在纤维化灶内有少许零星分布。结论肺纤维化与胶原Ⅰ、Ⅲ、Ⅴ型的过度沉积密切相关,观察Ⅳ型胶原在肺泡壁的分布变化对于发现早期肺损伤具有重要价值。Objective To investigate the abnomral deposition and distribution of types Ⅰ ,Ⅲ, Ⅳ, and Ⅴ collagen in rats with pulmonary fibrosis induced by bleomyein. Methods Pulmonary fibrosis was induced by intratracheal instillation of bleomyein in rats. The expressions of types Ⅰ ,Ⅲ, Ⅳ, and Ⅴ collagen were examined by immunohistoehemical staining and quantitatively assessed by image analysis Results Types Ⅰ , Ⅲ, and Ⅴ collagen showed overlapping distribution, and their contents increased with the progression of fibrosis. The inerease in the content of collagen 1 was the most significant, and the content of collagen Ⅲ also increased signifieandy. Alhough the content of collagen Ⅴ was low, but it increased slowly. Collagen Ⅳ, whieh was distributed in the basilar membrane of alveolar epithelial cells and capillary endothelium, showed marked rupture and deletion 3 days after the administration of bleomycin, and was distributed sporadically 28 days later. Conclusion Excessive disposition of types Ⅰ , Ⅲ, and Ⅴ collagen plays an important role in the puhnonary fibrosis. The distribution of type Ⅳ collagen in alveolar wall may help the early diagnosis of puhnonary injury.
