摘 要:目的:探讨菌尿对尿沉渣分析仪和尿干化学分析仪红细胞和隐血结果的影响;研究不同种类病原菌对尿红细胞和隐血结果的影响与发生机制。方法:取健康人尿液,加入一定浓度的常见病原菌制成人工菌尿标本(选取泌尿系统感染主要病原菌包括白假丝酵母菌、金黄色葡萄球菌、大肠埃希菌、铜绿假单胞菌、粪肠球菌),加入病原菌前后分别用仪器法进行红细胞计数和隐血结果测定。结果:自然菌尿和非菌尿红细胞检测,差异有显著性意义(P<0.01)。洁净尿经人工加入病原菌前后,尿红细胞数量比较差异有显著性意义,以白假丝酵母菌对仪器法尿红细胞计数影响最大(t=-22.17,P<0.01),其次是金黄色葡萄球菌(t=-9.758,P<0.01);加入病原菌前后,铜绿假单胞菌、大肠埃希菌和金黄色葡萄球菌三组尿液隐血差异有显著性意义,以铜绿假单胞菌对仪器法检测尿液隐血结果影响最大。结论:菌尿可影响全自动尿沉渣分析仪和干化学分析仪的红细胞和隐血检测结果,故尿液检验时应加强尿液的质量控制,减少菌尿对其干扰。Objective: To study the results of red blood cells (RBC) in bacteriuria measured by urinary sediment analyzer and dry chemistry analyzer,to study the effect of pathogenic bacteria types on the RBC in the urine,and to explore the underlying mechanism of this effect. Methods: 1. Use UF-100 and microscopy to count the RBC in bacteriuria and in nonbacteriuria. 2. Add some common pathogenic bacteria including Monilia albicans, Staphylococcus aureus, Escherichia eoli, Pseudomonas aeruginosa,Enteroeoccus faeealis) to healthy urine and then determine the RBC and occult blood in before and after the intervention. Results: A significant difference in the RBC counting was found between the outcomes of bacteriuria and nonbaeteriuria by UF-100 and microscopic examination with rank sum test ( P 〈 0.01 ). Before and after bacteria accession to the urine,the determination of urino-RBC and the result of occult blood were tested significant difference with pair t test. Candida albicans had the greatest impact on the RBC counts in the urine (t = -22.17 ,P 〈 0.01 ) ,followed by Staphylococcus aureus (t = - 9. 758, P 〈 0. 01 ). Pseudomonas aeruginosa, Eseherichia coli, and Staphylococcus aureus had statistically significant difference in occult blood using chi-square test, of which Pseudomonas aeruginosa had the greatest impact. Conclusion:Baeteriuria sediment can affect the results of the automatic dry chemistry analyzer and the urinary sediment analyzer. The quality control of urine,therefore,is important to the result evaluation.
