摘 要:目的探讨磁共振弥散加权成像(DWI)及表观弥散系数(ADC)在急性脑干梗死中的诊断价值。方法回顾性分析50例急性脑干梗死患者的MRI检查资料(均已行CT检查排除脑出血),其中超急性期(〈6h)1例,急性期(6~24h)6例,亚急性期(24~72h)43例,均行FLAIR T1WI、FLAIR T2WI、三维同性SE-EPI DWI序列扫描。测定病灶区ADC值及对侧镜像区ADC值做对照分析。结果①50例患者的DWI均能快速、敏感得显示位于脑干的急性梗死灶,明显优于常规FSLAIR T1WI、FLAIR T2WI序列。②超急性期、急性期、亚急性期病灶的ADC值均下降,各期病灶区ADC值及对侧镜像区ADC值比较均有显著统计学差异(P〈0.05)。结论三维同性SE-EPI DWI序列及ADC值在发现急性脑干梗死灶较常规MRI扫描及CT具有更高敏感性,能快速、准确诊断脑干急性梗死灶。Objective To study the role of isotropic diffusion weighted imaging (DWI) and apparent diffusion coefficient (ADC) in the diagnosis of acute brain stem infaretion. Methods MR findings of acute brain stem infarction in 50 cases, since Jan lth 2008 to Oct 17th 2008, were retrospectively analyzied. All of them had got negative results of brain hemorrhage in the exams of CT scanning. Among them, 1 cases were superacute stage (〈6h), 6 were acute stage (6--24h) and 43 eases were subacute stage (24-72h).AU the cases were examined with FLAIR T1WI, FLAIR T2WI and SE-EPI DWI. ADC value of infincted lesions and eontralateral same areas were measured and analyzied in every case respectively. Results (1)DWI can obviously delineate early ischemic lesions in brain stem. (2)ADC value of infineted lesions in every ease was reduced in comparison with that of contralateral same areas. There was significantly difference (P〈0.05). Conclusion DWI is more sensitive in the diagnosis of acute brain stem infarction compared with conventional MRI imaging methods and CT scanning.
