p53基因治疗晚期肝癌的初步生物学监测研究

摘  要:目的观察晚期肝癌患者动脉灌注人重组腺病毒p53介入治疗前后的生物学改变。方法采用流式细胞术、人体内微核实验检测介入治疗前后患者外周血中突变型p53表达及自发微核形成的改变。结果经p53基因动脉灌注介入治疗后,患者突变型p53平均表达率明显下降,和治疗前相比较(治疗前平均23.74%,治疗后平均11.81%)差异有统计学意义(P<0.01);介入治疗后患者的平均微核率(MNF)与治疗前的比较(0.144%vs 0.205%),差异有统计学意义(P<0.05)。结论动脉灌注人重组腺病毒p53介入治疗晚期肝癌中的生物学研究对治疗剂量的选择、治疗效果的判断有积极的指导和监测作用。Objective To investigate the biological changes after hepatic arterial perfusion of recombination adenovirus p53 gene ( rAd-p53 ) in the therapy of advanced hepatocellular caicinoma. Methods Flow cytometry (FCM) and micronucleus assay in vivo were used to detect p53 gene mutation and changes of spontaneous micronucleus(MN) formation in peripheral blood of 51 patients of HCC. Results The depression of p53 expression was observed in the HCC patients, the differences of p53 expression between before and after interventional tAd-p53 therapy (23, 74% vs 11.81% ) were statistically significant ( P 〈 0.01 ). The MNF depressed by the rAd-p53 therapy were seen in the patients, and the differences of the MNF between before and after interventional rAd-p53therapy were statistically significant (P 〈 0.05 ). Conclusions Biological study of interventional therapy which hepatic arterial perfusion of rAd-p53 in the advanced HCC will play an important role in selecting the therapeutic dose and evaluating therapeutic efficacy, by means of guiding and monitoring.
