肾血管平滑肌脂肪瘤CT诊断与鉴别诊断

摘　　要：目的:探讨肾脏血管平滑肌脂肪瘤(RAML)的CT征象及鉴别诊断。方法:回顾性分析26例经CT平扫及增强扫描并经手术病理证实的RAML的临床及CT资料,扫描设备为SOMATON SPIRIT双层螺旋CT扫描仪。结果:CT表现为肿瘤呈圆形或类圆形,密度不均匀,有软组织及脂肪等多种成分或以某种成分为主。2例病灶可见斑片状钙化,2例巨大肿瘤病灶合并出血。增强扫描表现为病灶不同程度强化,脂肪成分不强化,部分病例可见条索状和斑点状强化影。结论:典型的RAML有其特征性的CT征象,病灶内是否含有脂肪组织是确诊的重要依据,增强扫描肿块内可见条索状强化影,颇具特征性。巨大肿瘤病灶可合并出血。Objective：To study the renal angiomyolipoma（RAML） of the CT signs and differential diagnosis. Methods： Retrospective analysis of 26 cases by CT scan and scan and confirmed by surgery and pathology＇s RAML of clinical data and CT scanning equipment for SOMATON SPIRIT double-spiral CT scanners. Results： CT showed the tumor was round or round, uneven density, soft tissue and fat and other components or the main ingredient in some, 2 cases of calcified lesions can be seen patchy, and 2 eases of huge minor with bleeding lesions. Scan showed lesions in varying degrees to strengthen, not fat enhanced, we can see some of the cases of cord-like spots and video-enhanced, Conclusion： RAML has its typical characteristics of the CT signs of whether the lesions containing fat tissue is an important basis for diagnosis, CT can be seen inside the tumor-like cord to strengthen video, rather characteristic. A huge tumor bleeding lesions can be combined.
