文拉法辛对伴抑郁焦虑的非糜烂性胃食管反流病的作用

摘　　要：目的:探讨文拉法辛对伴有抑郁和/或焦虑的非糜烂性胃食管反流病的治疗效果.方法:56例伴有抑郁和/或焦虑的非糜烂性胃食管反流病患者双盲随机分为对照组和治疗组,所有患者使用埃索美拉唑(20mgqd)联合莫沙必利分散片(5mgtid)治疗4wk;治疗组另加用文拉法辛(25-50mgtid)治疗4wk.观察烧心、反流、抑郁和焦虑等症状,并用症状积分及有效率评价疗效.结果:与治疗前比较,2组患者治疗后烧心、反流积分均改善,差异有统计学意义(P<0.01);治疗后治疗组与对照组比较,患者烧心、反流积分均改善,差异有统计学意义(P<0.01),HAMD及HAMA积分均改善,差异有统计学意义(P<0.05);治疗组总有效率高于对照组(96.1%vs79.1%,P<0.05).结论:对伴有抑郁和/或焦虑的非糜烂性胃食管反流病文拉法辛和埃索美拉唑及莫沙必利联用有较好的疗效.AIM： To investigate the efficacy of Venlafaxine in treatment of non-erosive gastroesophageal reflux disease accompanied with depression and anxiety. METHODS： Fifty-six patients with non-erosive gastroesophageal reflux disease accompanied with depression and anxiety were divided into treatment group and control group double-blind- ly and randomly. All patients were treated with Esomeprazole （20 mg qd） and Mosapride dispers- ible tablet （5 mg rid）. Patients in treatment group were treated with Venlafaxine （25-50 mg tid）. Two groups were under treatment for 4 weeks. The symptoms of heartburn, contraflow, depres- sion and anxiety were observed before and after treatment. The therapeutic efficacy was evaluated by the symptom scores and effective power.RESULTS： The difference of heartburn scores and contraflow scores before treatment, was significant between and within the two groups （P 〈 0.01）. After treatment, the differences of heartburn, contraflow scores, Hamilton Depres- sion Scale （HAMD） and Hamilton Anxiety Scale （HAMA） were also significant （P 〈 0.05）. The to- tal effective rate of treatment group was higher than that of control group （96.1% vs 79.1%, P 〈 0.05）. CONCLUSION： The therapeutic efficacy of com- bined treatment of Venlafaxine, Esomeprazole and Mosapride is good for non-erosive gastro- esophageal reflux disease accompanied with depression and anxiety.
