榄香烯乳剂联合化疗治疗晚期非小细胞肺癌的临床观察

摘　　要：目的:观察榄香烯乳剂联合化疗治疗晚期非小细胞肺癌的疗效与安全性。方法:将62例晚期非小细胞肺癌患者随机分为A组(榄香烯注射液+化疗,32例)和B组(单纯化疗30例);比较两组治疗前后肿瘤病灶、生活质量、T细胞亚群、血清癌胚抗原(CEA)、糖链抗原-125(CA-125)水平和不良反应发生情况。结果:2周期治疗结束后,A组生活质量评分较B组明显改善(P<0.05)、A组有效率(31.25%)较B组(26.67%)明显升高(P<0.05);A组治疗后较治疗前CD3+、CD4+细胞、CD4+/CD8+比值升高(P<0.01);CD8+细胞明显下降。而B组治疗后患者CD3+、CD4+细胞、CD4+/CD8+比值较治疗前降低(P<0.05);CD8+细胞升高,但无统计学差异。与治疗后B组相比,A组血清CEA、CA-125明显减低,(P<0.01);B组治疗后CEA、CA-125也较治疗前减低,但无统计学差异。不良反应A组较B组减轻。结论:榄香烯注射液联合化疗可改善晚期非小细胞肺癌患者的生活质量,提高疗效。Objective： To observe the effects of elemene emulsion combined with chemotherapy in treatment of patients with ad vanced non small cell lung caneer（NSCLC）. Methods：The clinical data of 62 patients with advanced NSCLC from March 2005 to June 2007 were randomly assigned into Group A （32 cases,were treated with combined method） and Group B were treated with chemotherapy only. The tumor size, life quality,T lymphocyte sub-group lever, serum CEA,CA-125 level and side effects were observed before and after treatment in both groups. T lymphocyte sub group level was detected by flow-cytometry（FCM）. Serum CEA and CA-125 level were examined by ELISA method. Results： After two cycles treatment, Karnofsky scores of group A were increased significantly compared with Group B （P〈0.05）. The effective rates in Group A and Group B were significantly different（31. 25% vs 26. 67%）. Compared after treatment and before treatment, CD^ ＋,CD4^＋ cells and the ratio of CD4^＋/ CD8^＋ increased significiently （ P〈0. 01 ） ; In contrast, CD8^＋ cells decreased markedly（ P〈0.05 ）. However, the changes of CD3 ＋ ,CD4 - ,CD8 - cells and the ratio of CD4^＋/ CD8^＋ of Group B were contrary to those of Group A. After 2 cycles treatment. the levels of serum CA 125 and CEA were decreased significantly in Group A （both P〈0.01）; there were also decreased in Group B （P〉0.05）. A lower toxicity side effects were observed in Group A than in Group B. Conclusion：Elemene emulsion combined with chemotherapy may make the life quality become better and increase the treatment effects of patients with ad vanced non-small cell lung cancer.
