摘  要:目的建立一种简单、稳定的大鼠肺缺血再灌注损伤(LIRI)长期存活模型。方法将84只健康SD大鼠随机分为2组:Ⅰ组为假手术组,Ⅱ组为肺缺血再灌注组(每组n=42只)。两组分别于开胸后、缺血1h后再灌注0、2、4h、1、3、7d取肺组织行髓过氧化物酶(MPO)活性、肺湿干比(W/D ratio)检测和肺泡Ⅱ型细胞(ATⅡ)的电镜超微结构评价;取支气管肺泡灌洗液检测肺通透性指数(LPI)。结果手术成功率达100%。Ⅱ组与Ⅰ组比较,缺血再灌注后2、4h、1d的MPO、LPI、W/D比差异均有统计学意义(P〈0.01),开胸后、再灌注后0h、3、7d两组比较差异无统计学意义(P〉0.05)。ATⅡ的超微结构显示,再灌注4h后损伤最严重,再灌注1d即出现明显的修复过程,再灌注7d基本恢复正常结构水平。结论本模型简单、可靠,LIRI后相关肺损伤指标和ATⅡ超微结构损伤变化特点吻合。Objective To establish a simple and stable rat lung ischemia-reperfusion (LIRI) long-term survival model. Methods Eighty-four healthy SD rats were randomly divided into two groups : Group Ⅰ,the sham-operated group (n = 42, no hilar blocking) ;Group Ⅱ, lung ischemia reperfusion group (n = 42,left hilar blocking). The lung tissues and blood samples were collected at baseline before hilar occlusion,and 0 h,2 h,4 h,1 day,3 days and 7 days after reperfusion. The indices were determined as follows : the myeloperoxidase (MPO) activity, wet/dry ratio ( W/D ratio) ,lung permeability index (LPI) and the uhrastructure of AT Ⅱ. Results All the SD rats survived during the observation period. Compared with group Ⅰ, the MPO activity, W/D ratio, and the LPI were significantly increased in group II at the time points of 2 h,4 h, 1 day after reperfusion ( all P 〈 0.01 ). The uhrastructure of AT Ⅱ displayed that the most serious injury occurred at the tine point of 4 h after reperfusion. The repair process emerged at 1st day after reperfusion. At 7th day after reperfusion, the lung tissue structure almost recovered to normal. Conclusion It is a simple and reliable animal model. The related indices of lung injury were in accordance with the changes of uhrastruture of AT Ⅱ.
