摘 要:目的:观察活血通脉利水明目之散血明目片对非缺血型视网膜静脉阻塞患者视网膜电图和视野的影响,探讨治疗机制。方法:34例非缺血型视网膜静脉阻塞患者随机分为治疗组(17例17眼)与对照组(17例18眼),治疗组治以散血明目片联合中西医常规治疗方法;对照组治以血栓通片联合中西医常规治疗方法。治疗1疗程后检测ERG与电脑视野。结果:经活血通脉利水明目法治疗后,Ops中O4子波的波峰时值未发生延迟(P>0.05);治疗组治疗后暗视ERGa波波幅为(173±25.9)μV,ERGb波波幅为(349±34.6)μV,Ops中O2子波波幅为(112.5±12.5)μV,Ops中∑O波幅为(174.1±30.7)μV,与对照组治疗后比较有较大提高,差异有显著性或非常显著性意义(P<0.05,P<0.01);治疗组治疗前后与对照组治疗前后视野扩大均值比较,差异有非常显著性意义(P<0.01);治疗组患者的视野敏感度于中心10°视野平均敏感度恢复最好,与治疗前比较,差异有非常显著性意义(P<0.01),而对照组治疗后视野各个区域平均敏感度均无明显改善(P>0.05)。结论:活血通脉利水明目法(散血明目片)是治疗非缺血型视网膜静脉阻塞的有效方法,其作用机制可反映在患者视网膜电图与视野变化上。Objective:To observe the effect of Sanxue Mingmu Tablets(SMT),with the actions of promoting urination and improving eyesight,on electroretinogram(ERG) and visual field in patients with non-ischemic retinal vein obstruction,and to explore its therapeutic mechanism. Methods:Thirty-four qualified patients were equally randomized into the treatment group(involving 17 eyes) and the control group(involving 18 eyes). The two groups received routine western and Chinese medical treatment. Additionally,the treatment group was given SMT,and the control group received Xueshuangtong Tablets. After one treatment course,ERG and computer-based visual field were measured. Results:After treatment,wave crest time of O4 wavelet of oscillatory potentials(Ops) was not delayed in the treatment group(P > 0.05). The amplitude of wave a in ERG was(173±25.9)μV,amplitude of wave b was(349±34.6)μV,amplitude of wavelet O2 in Ops was(112.5±12.5)μV,amplitude of wave ∑O in Ops was(174.1±30.7)μV in the treatment group,and the above indexes were increased obviously as compared those in the control group(P < 0.05 or P < 0.01). The difference of the increase of visual field was significant in both groups before and after treatment(P < 0.01). The visual sensitivity at the central 10° of visual field was well recovered in the treatment group(P < 0.01 compared with other regions of the visual field),but the difference of visual sensitivity in various regions of the control group was insignificant(P > 0.05). Conclusion:SMT is effective for the treatment of non-ischemic retinal vein obstruction,and its therapeutic mechanisms may be related with the improvement of ERG and visual field.
