经筋排刺法配合皮肤针疗法治疗脑卒中后上肢痉挛临床研究

摘　　要：目的:观察经筋排刺法配合皮肤针疗法治疗脑卒中后上肢痉挛的临床疗效。方法:150例患者随机分为2组,在必要的康复治疗基础上,治疗组75例采用经筋排刺法配合皮肤针治疗;对照组75例采用常规西药治疗。于治疗前后采用改良的Ashworth痉挛量表(MAS)、简化的Fugel-Meyer运动功能评定量表(FMA)、改良Barthel指数(MBI)分别对2组患者进行上肢痉挛程度、上肢运动功能、日常生活活动能力进行评定。结果:2组患者治疗后肌张力均有明显改善(P<0.05),其中治疗组明显优于对照组,差异有显著性意义(P<0.05)。对照组总有效率为62.7%,治疗组为94.7%,2组比较,差异有显著性意义(P<0.05),治疗组明显优于对照组。治疗后治疗组FMA、MBI评分明显较对照组高,差异有显著性意义(P<0.05),治疗组上肢运动功能及日常生活活动能力改善更显著。结论:经筋排刺配合皮肤针疗法治疗脑卒中后上肢痉挛的临床疗效优于常规药物治疗。Objective:To observe the therapeutic effect of muscle-along-meridian row-needling therapy combining dermal needling therapy for the treatment of post-stroke upper limb spasm. Methods:One hundred and fifty qualified patients were equally randomized into the treatment group and the control group. Both groups received rehabilitative treatment. Additionally,the treatment group was given muscle-along-meridian row-needling therapy combining dermal needling therapy,and the control group received routine western medicine. The scores of upper limb spasm,upper limb movement and activities of daily living(ADL) were measured by Modified Ashworth Scale(MAS),simplified Fugel-Meyer Assessment(FMA) and modified Barthel Index(MBI) in both groups before and after treatment. Results:After treatment,muscular tension was improved in both groups(P < 0.05),and the improvement was obvious in the treatment group(P < 0.05). The total effective rate was 62.7%in the control group and 94.7%in the treatment group(P < 0.05). The scores of FMA and MBI were higher in the treatment group than those in the control group(P < 0.05),and the improvement of upper limb movement and ADL was obvious in the treatment group. Conclusion:Muscle-along-meridian row-needling therapy combining dermal needling therapy has a better effect than routine western medicine for the treatment of post-stroke upper limb spasm.
