摘  要:【目的】探讨肝细胞癌外周血AFP mRNA表达水平与早期微转移的关系。【方法】用0.2%二己基亚硝胺(DEN)诱导Wistar大鼠建立肝癌模型,提取外周血总RNA,经转录后,采用巢式反转录聚合酶链反应(巢氏RT-PCR)技术检测大鼠AFP mRNA水平。【结果】20只对照组大鼠外周血标本中未检测到AFP mRNA,而在79只存活的实验组大鼠中,肝硬化组AFP mRNA检出率为6.3%(1/16),未转移肝癌组AFP mRNA检出率为42.3%(22/52),伴转移肝癌组AFP mRNA检出率为81.8%(9/11)。【结论】以AFP mRNA为标志物,通过巢氏RT—PCR来检测肝细胞癌外周血微转移是可行的,有助于提高肝细胞癌外周血微转移检测的敏感性和特异性。早期检测外周血微转移可以提高预测肝细胞癌复发、转移的能力。[Objective]To analyze the relationship between the expression of AFP mRNA in peripheral blood and its micrometastasis in Wistar rats with hepatoce/lular careinoma(HCC). [Methods]Wistar rat model of hepatocellular carcinoma was induced by 0.2 % diethylinitrosamine (DEN). Total RNA from the peripheral blood of rats was extracted and AFP mRNA in peripheral blood of rats was detected by nested reverse tran scription polymerase chain reaction (RT-PCR). [Results]None of the control group showed the expression of AFP mRNA in peripheral blood. In 79 live rats with HCC, the positive rates of the expression of AFP mRNA were 81.8% (9/11) in the group of HCC (with metastasis), 42.3% (22/52) in the group of HCC (without metastasis) and 6.3% (1/16) in the group of liver cirrhosis. [Conclusion]Nested RT-PCR is a feasible method for detecting circulating tumor cells in peripheral blood with a liver-specific AFP mRNA marker, which can increase the sensitivity and specificity of detection of micrometastasis in HCC. Moreover, the early detection of micrometastasis can increase the predictable ability of recurrence and metastasis in HCC.
