导管持续引流联合鸦胆子油乳、顺铂治疗恶性胸腔积液的疗效观察

摘　　要：目的观察导管持续引流联合鸦胆子油乳、顺铂治疗恶性胸腔积液的疗效及毒副作用。方法将120例恶性胸腔积液患者随机分为治疗组和对照组,每组60例,留置导管排尽胸水后,治疗组予鸦胆子油乳100ml+顺铂40mg/m2胸腔内注入;对照组予顺铂40mg/m2胸腔内注入。根据胸水情况,5～7d治疗1次,共4次,观察临床疗效、生活质量及毒副作用。结果治疗组有效率、生活质量改善率高于对照组,化疗毒副反应低于对照组,两组比较差异均有统计学意义(P<0.05)。结论导管持续引流联合鸦胆子油乳、顺铂治疗恶性胸腔积液的疗效显著、毒副作用轻,值得推广。Objective To observe the clinical curative effect and side effect of catheter constant drainage combined brucea javanica oil emulsion （BJOE） and cisplatinum on treating malignant pleural effusion. Methods 120 cases with malignant pleural effusion were randomized into two groups,catheterized and drained after thoracocentesis. The treatment group in which 60 cases were given brucea javanica oil（100ml） plus cisplatinum（40mg/m2） by intrathoracic injection,while control group in which 60 cases were given cisplatinum（40mg/m2） by intrathoracic injection. The therapies were repeated every five to seven days according to the situation of pleural effusion for a total of four times.The target of observation were clinical efficacy,quality of life,and side-effects.Results The effective rate and improvement rate of life quality in treatment group were higher than that in control group,while the side-effect of chemotherapy was lower. There were striking difference（P〈0.05）between two groups.Conclusion With malignant pleural effusion,catheter constant drainage combined injecting BJOE and cisplatinum into thoracic cavity was proven to be a higher effect rate treatment method with lower toxicities.It deserves widely clinical application.
