中西医结合非手术治疗上消化道溃疡穿孔疗效观察  

Observation of Therapeutic Effect on Gastroduodrnal Ulcer Perforation with Non-Operative Treatment of Combination of TCM and Western Medicine

摘　　要：目的:比较中西医结合非手术治疗与手术治疗上消化道溃疡穿孔的临床疗效。方法:137例中,82例用中西医结合非手术治疗,55例用手术治疗。结果:中西医结合非手术治疗成功81例,中转手术1例,全部痊愈,无并发症发生,1年后溃疡愈合率93.2%,随访无再次穿孔。手术治疗痊愈54例,死亡1例,切口感染2例,肠梗阻1例,1年后溃疡愈合率94.3%。两者之间治愈率、溃疡愈合率还是并发症发生率均无显著差异,但非手术治疗组住院天数、医疗费用明显降低(P<0.05)。结论:中西医结合非手术在治疗上消化道溃疡穿孔上能取得等同于手术治疗的临床疗效,非手术治疗是安全有效、简便易行、费用较低的治疗手段。Objective：To compare clinical effect of non - surgical therapy of traditional chinese medicine integrated with western medicine with that of surgical therapy in treating patients suffered from peptic ulcerous perforation. Methods：From October 2005 to October 2008, the clinical data of 137 patients with peptic ulcerous perforation was analyzed retrospectively. Result： of the 82 patients treated by non - surgical therapy, 81 were healed successfully, with 1 case transferred to operation. No complications happened in all of the 82 patients. Peptic ulcer disappeared in 93.2% of the 82 patients one year later and no relapse happened during follow - up. Of the 55 cases treated by operation, 54 were cured,2 suffered from incision infection postoperatively, 1 suffered from intestinal obstruction, and 1 died. Peptic ulcer disappeared in 94.3% of the 55 patients during follow - up for one year. There was no significant difference between two groups in the rate of peptic ulcer disappearance and complication, however, the expenditure and the date in hospital was lower in non - surgical group （P 〈0. 05 ）. Conclusion： Non - surgical therapy has the similar therapeutic effect in treating peptic ulcerous perforation, compared with that of surgical therapy. Non - surgical therapy possesses advantages such as safety, effectiveness, convenience and low expenditure, and can be accepted by patients more easily.
