摘 要:目的探讨微血管密度(MVD)、DNA倍体、免疫表型与前列腺上皮内瘤(PIN)及其生物学特性的关系。方法应用免疫组织化学SP法和计算机图像分析技术,对24例前列腺增生症(BPH)、23例低级别上皮内瘤(LGPIN)、26例前列腺癌(PC)伴发高级别上皮内瘤(HGPIN)的石蜡包埋标本进行CD34、34βE12、P63、P504s表达的检测,定量测定其MVD和细胞核DNA倍体。结果不同级别的PIN中MVD和DNA平均倍体均明显高于BPH组,但明显低于PC组(P<0.01),且随PIN级别的增高,DNA倍体的异多倍体数及MVD值随之升高(P<0.01)。DNA倍体与MVD呈正相关关系。34βEl2和P63在BPH和LGPIN腺体周围的基底细胞呈连续阳性,在HGPIN中呈断续阳性,在PC中呈阴性。P504s在HGPIN中呈灶状阳性,在PC中呈强阳性,在BPH和LGPIN中呈阴性。结论DNA倍体、MVD与PIN密切相关,是反映其生物学特性的重要参考指标;免疫表型进一步证实PIN特别是HGPIN是PC的癌前病变。Objective To study microvascular density (MVD), DNA ploidy and immunophenotype in prostatic intraepithelial neoplasia (PIN) as well as their relationship with its biological properties. Methods We studied paraffin-embedded specimens from 24 cases of benign prostatic hyperplasia (BPH), 23 cases of low-grade prostatic intraepitheliat neoplasia (LGPIN), and 26 cases of prostatic carcinoma (PC) with high-grade prostatic intraepithelial neoplasia (HGPIN). Image analysis was used to measure the DNA average ploidy and MVD, and after that the expressions of 34~E12, P63, P504s and CD34 were detected by immunohistochemistry SP. Results DNA ploidy and MVD in PIN were higher than those in BPH, but lower than in PC, and correlation was detected among different grades of PIN. MVD had a positive correlation with DNA aneuploidy in different grades of PIN (P〈O. 01), and there was a significant positive correlation between expression of DNA ploidy and MVD. Acini of PC were completely negative for both basal cell markers (34~3E12 and p63), but were positively different in the BPH and among different grades of PIN, which were in contrast to the results for the acini cells of revealed varying degrees of expression of PS04s in the PC and HGPIN, and acini of BPH and LGPIN were negative. Conclusion MVD and DNA ploidy were correlated to the biological properties of PIN, and thus were important parameters. HGPIN is a precancerous change of prostatic carcinoma by expression of MVD and DNA ploidy and these immunophenotypes.
