摘　　要：目的探讨热休克蛋白27(HSP27)在胃癌和癌前疾病中的表达及临床意义。方法应用免疫组织化学SABC法检测120例经病理证实的健康胃黏膜组织、胃溃疡、慢性萎缩性胃炎和胃癌4种不同胃黏膜病变中HSP27的表达情况,通过图像分析软件对HSP27的平均吸光度值进行测定,并对临床相关资料进行统计学分析。结果HSP27在健康胃黏膜组织(0.292±0.012)和胃溃疡(0.302±0.011)中较慢性萎缩性胃炎(0.305±0.010)的表达低,慢性萎缩性胃炎与健康胃黏膜组织比较差异有统计学意义(P<0.05)。胃癌(0.313±0.024)的表达较前三者高,差异有统计学意义(P<0.05)。HSP27的表达与性别、年龄、组织类型和浸润深度无关;而与分化程度、淋巴转移有关(P<0.05)。结论HSP27与肿瘤的发生有关,检测该指标可为胃癌诊断、判断预后和指导治疗提供理论依据。Objective To investigate the expression of heat shock protein 27 （HSP27） in gastric cancer and precancerous ailments and its possible clinical significance. Methods The expression of HSP27 was studied by immunohistochemical SABC method in 120 gastric specimens including 20 normal groups, 22 gastric ulcers （GU）, 24 chronic atrophic gastritis （CAG） and 54 gastric cancers （GC）. The average light density was assayed by the expression of HSP27, and the statistic software was used to analyze the clinical data correlated. Results The average light density of HSP27 in normal group and gastric ulcer was lower than that in chronic atrophic gastritis. Compared with normal group, significantly higher HSP 27 was detected in chronic atrophic gastritis（P〈0.05）. The expression of HSP 27 in gastric cancer was higher than the others（P〈0.05）. The expression of HSP27 was negatively correlated to age, gender, histological type and the depth of tumor invasion（P〈0.05）, but significantly associated with the cellular differentiation and lymph node metastasis（P〈0.05）. Conclusion The HSP27 may play an important role in carcinogenesis and progression of gastric carcinoma. The detection of HSP27 can provide theoretical basis for diagnosing, evaluating prognosis and conducting cure of gastric cancer.
