微血管减压术治疗面肌痉挛58例报告

摘　　要：目的:探讨面肌痉挛微血管减压术的手术技巧和并发症的预防。方法:回顾性分析58例面肌痉挛病人手术治疗的资料,改进责任血管的隔离技术。结果:53例术后即缓解,4例延迟治愈,1例部分缓解。总并发症9例,其中神经并发症7例(12.1%),手术相关并发症2例。结论:微血管减压术可以取得较好的疗效,是目前有效的治疗方法,微血管减压术是微创技术在功能神经外科应用的典范之一。Objective： To report the treatment of 58 cases of hemifacial spasm by microvascular decompression.Methods： A total of 58 patients were treated surgically between year 2003 and 2008.The clinical symptoms,operative techniques and complications were investigated.Results： Of the 58 cases,53 cases were spasm-free immediately after surgery,4 cases experienced delayed resolution of residual hemifacial spasm,and only one patient resolved partially.The neurological complications（12.1%） included vertigo,reduction in auditory acuity,facial weakness,and tinnitus.Two cases suffered from cerebrospinal fluid leakage.The total rate of complications were 15.5%.Conclusion： Microvascular decompression is an effective procedure to treat hemifacial spasm in functional neurosurgery.
