上皮钙粘附蛋白在甲状腺乳头状癌中的表达与血清TSH浓度相关性的临床研究

摘　　要：目的研究甲状腺乳头状癌中上皮钙粘附蛋白(E-cadherin)的表达与血清TSH浓度的相关性,探讨E-cadherin在甲状腺乳头状癌中的作用及E-cadherin与血清TSH浓度在甲状腺乳头状癌中的相互作用。方法回顾性分析30例甲状腺乳头状癌患者血清TSH浓度,手术切除标本病理切片免疫组化染色后E-cadherin的表达情况。结果(1)E-cadherin在不同性别、年龄、肿瘤大小的甲状腺乳头状癌患者之间的阳性表达率差异无显著性(P>0.05);(2)E-cadherin阳性表达率与血清TSH浓度1组(rs=0.287,P<0.05)、2组(rs=0.298,P<0.05)、3组(rs=0.474,P<0.05)均呈正相关,且E-cadherin的阳性表达率随着血清TSH浓度升高而增加。结论甲状腺乳头状癌组织中E-cadherin的表达阳性率与血清TSH浓度之间存在正相关,血清TSH可增进E-cadherin在甲状腺乳头状癌中的表达。探讨二者在甲状腺乳头状癌中的作用与相互关系,对甲状腺乳头状癌患者的治疗及预后判断有深远的意义。Objective To research the function of E-cadherin in papillary thyroid carcinoma and the interaction between E-cadherin and serum Thyroid-Stimulating Hormone （TSH） concentration through investigated the the relativity between the expression of E-cadherin in this kind of cancer and serum TSH concentration. Methods 30 patients with papillary thyroid carcinoma were retrospectively analyzed in serum TSH concentration and expression of E-cadherin in postoperative pathological specimen by immunohistochemical experiment. Results There were no statistical significantly differences the positive expression rate of E-cadherin among the gender, age and tumor size of the patients with papillary thyroid carcinoma （P〉0.05） ; The positive expression rates of the E-cadherin in patients with papillary thyroid carcinoma were positive relativity with the serum TSH concentrations of group 1 （r= 0.287,P 〈0.05）, group2 （ r =0.298, P 〈0.05）, andgroup3 （r=0.474, P〈0.05） , Which the positive expression rate of E-cadherin was increased as the serum TSH concentration. Conclusion The positive expression rates of E-cadherin in papillary thyroid carcinoma are positively correlated with serum TSH. Serum TSH can increase the expression of E-cadherin in the cancer. There are far-reaching meaning to study the ralationship between TSH and E-cadherin in treatment and prognosis of papillary thyroid carcinoma.
