胆囊腺瘤癌变过程及免疫表型P53的诊断价值

摘　　要：目的探讨胆囊腺瘤的癌变过程及P53的诊断价值。方法利用免疫组织化学S-P法检测40例胆囊腺瘤P53蛋白的表达情况。结果23例胆囊腺瘤伴有上皮内瘤变,其中伴有低级别上皮内瘤变12例,高级别上皮内瘤变11例。伴有低级别上皮内瘤变1例P53弱表达。伴高级别上皮内瘤变8例P53过表达,其中5例局部呈现原位癌,3例重度非典型增生。结论胆囊腺瘤的大小与上皮内瘤变的程度及癌变有相关性趋势,P53蛋白的表达水平可以反映上皮内瘤变的程度。Objective To study the P53 protein expression and its diagnostic significance during the malignant transformation of gallbladder adenoma. Method the expression of P53 protein was detected by S-P immunohistochemistry in 40 cases of gallbladder adenomas. Results Gallbladder adenomas with intraepithelial neoplasia were 23 ,including 12 cases of low grade intraepithelial neoplasia and 11 cases of high grade intraepithelial neoplasia. Inununiobistochemical results showed that P53 overexpression was observed in 1 case of low grade intraepithelial neoplasial 8 cases in high grade intraepithelial neoplasia. Conclusion The tumor size is closely linked to the malignant transformation potential of gallbladder adenoma. The P53 protein expression may play a diagnostic role in the adenoma-intraepithelial neoplasia-carcinoma of the gallbladder.
