秋田注气结扎术与Super 7圈套结扎术治疗食管静脉曲张比较

摘　　要：目的探讨秋田注气及附加气囊MD-48720结扎术与Super 7结扎术治疗食管曲张静脉的优缺点。方法69例肝硬化食管静脉曲张破裂出血病人分成两组,A组39例行秋田注气结扎治疗,B组30例行Super 7圈套结扎治疗,2周重复1次,直至静脉曲张消退,对比研究两者治疗特点及并发症等。结果两组治疗成功率和消退曲张静脉方面差异无统计学意义(P>0.05),秋田结扎术单次和总平均所需费用明显低于Super 7圈套结扎术(P<0.01),但秋田结扎术单次治疗所需时间长于Super 7圈套结扎术(P<0.01)。结论秋田注气及附加气囊MD-48720结扎器结扎食管曲张静脉在安全性、并发症处理方面均优于Super 7圈套结扎治疗,可根据静脉曲张情况选用环数,费用低;而Super 7圈套结扎术操作更显方便,省时,患者耐受性好,但价格昂贵,浪费大。Objective To study the difference between Akida ligation and Super 7 ligation in the treatment of esophageal vafices. Methods Sixty-nine patients with cirrhosis who had ever bled for esophageal varices were randomized to undergo either Akida ligation （ n = 39 ） or Super 7 ligation （ n = 30 ）. Treatments were held at 2-week intervals until the eradication of the varices. The characteristics of the treatment and complications between two techniques were compared. Results The rates in success and eradicating varices in both groups were similar （ P 〉 0. 05 ）. The once or all cost of Akida ligation was significandy lower than that of Super 7 ligation （ P 〈0. 001 ）. However, the time of a single treatment was longer in the group of the Akida ligation （ P 〈 0. 05 ）. Conclusion The Akida ligation is superior to the Super 7 ligation in sides of safety and treatment of complication. Doctor can choose to use any numbers of rings freely in the Akida ligation according to patients＇ levels of varices, and the cost was lower. Although super 7 ligation is more convenient, patients have to pay more money for the treatment.
