腹腔镜手术在治疗胃肠道间质瘤中的应用研究

摘　　要：目的探讨腹腔镜下胃肠道间质瘤切除术的可行性与安全性。方法回顾性分析华中科技大学附属协和医院腹腔镜外科中心于2003年6月～2008年12月间确诊并施行了腹腔镜下胃肠道间质瘤切除术的42例患者临床资料。结果42例患者均成功施行了腹腔镜下间质瘤切除术,其中胃间质瘤32例,小肠间质瘤9例,直肠间质瘤1例,与之相应,手术时间分别为:(68±32)、(80±28)和140min,术中出血为:(50±23)、(80±33)和100mL,术后住院天数为:(6±2.5)、(7±2)和8d。随访3～72个月,1例小肠间质瘤患者复发且伴肝脏转移。结论腹腔镜下胃肠道间质瘤切除术是治疗GIST安全有效的术式选择。[ Objective ] To evaluate the feasibility and safety of laparoscopic resection of gastrointestinal stromal tumors （GISTs）. [Methods] Clinical data of 42 patients being diagnosed as GISTs and undergoing laparoscopic resection between June 2003 and December 2008 were analyzed retrospectively. [ Results ] All the tumors of the patients including 32 gastric GISTs, 9 small bowel GISTs and 1 rectum GIST had been successfully reseeted under laparoscope. The mean operative time was （68±32） min for gastric GISTs, （80±28） min for small bowel GISTs and 140 min for rectum GIST. The mean blood loss was （50±23） mL for gastric GISTs, （80±33） mL for small bowel GISTs and 100 mL for rectum GIST. The mean hospital stay was （6±2.5） days for gastric GISTs, （7±2） days for small bowel GISTs and 8 days for rectum GIST. At a follow-up of 3-72 months, one small bowel GIST had postoperative recurrence and liver metastasis. [ Conclusions ] Laparoseopic resection is a safe and feasible choice of treating GISTs.
