急性脑梗死偏瘫患者足底压力测定的临床研究

摘　　要：目的研究急性脑梗死（ACI）偏瘫患者足底压力测定的临床应用价值。方法对21例ACI偏瘫患者,采用德国Zebris测力板,于治疗前、治疗后30 d进行足底压力测试,记录足底各区压力值。结果治疗前患侧足底各区压力明显小于健侧（均P〈0.01）,与治疗前比较,治疗后患侧第1跖骨、足跟内侧、第3跖骨和足弓部压力明显升高（P〈0.05~0.01）。治疗后健侧足弓部压力峰值显著下降（P〈0.01）,而足跟内侧与治疗前比较压力显著下降（P〈0.05）,治疗后包络椭圆方向角显著改变（P〈0.01）,压力中心水平偏离参数较治疗前明显提高（P〈0.05）,轨迹总长度与轨迹包络面积的比值（1.39）比治疗前（1.65）明显下降。结论足底压力测量技术能客观、定量地评价ACI患者站立和行走功能,全面反映ACI患者治疗后步行的动力学状况,对指导ACI康复治疗及疗效判断有意义。Objective To investigate the clinical appliance value of plantar pressure measurement in hemiplegic patients with acute cerebral infarction（ACI）. Methods Plantar pressure was measurered and recorded by the means of Germany-made Zebrls in 21 hemiplegie patients with ACI before and 30 d after treatment. Results Although plantar pressure on foot of the paralysed side was found remarkably decreased compared to that of the contralateral side before treatment（ all P 〈 0. 01 ）, pressure of the first and third metatarsal bones, instep, as well as inner side of the heel bone showed greatly improved after treatment （P 〈 0. 05 - 0.01 ）. Peak pressure of the contralateral instep revealed notably decreased after treatment （P 〈 0. 01 ） , while that of inner heel bone showed moderately lower （ P 〈 0.05） ,ellipse direction angle exhibited apparently altered following the therapy （P 〈 0. 01 ）, leaning parameter of pressure center showed significantly elevated（P 〈0. 05 ）, and the ratio between trail length and area inside the trail （1.39） prominently lower than which （ 1.65 ） before treatment. Conclusions Plantar pressure measurement may provide an objective and quantitative evaluation on standing and walking function of patients with ACI as well as a thorough post-treatment judgement of motor function,and is meaning for direction of rehabilitation and curative effect judgement.
