摘 要:目的超声测量正常人及原发性开角型青光眼(POAG)不同发展阶段的视神经直径(OND),分析比较各组有无差异,探讨该技术的准确性与可行性。方法POAG患者115只眼,正常对照组40只眼,通过自动视野检查将POAG患者再分为视野损害前期组(PP)、POAG早期组(EP)、中期组(MP)、晚期组(LP)。检测五组的年龄、视力(矫正)、眼压、C/D、OND并进行对比分析。结果正常对照组的OND为(3.02±0.35)mm,POAG各组OND依次为(2.96±0.29)mm、(2.80±0.41)mm、(2.73±0.19)mm、(2.62±0.29)mm。五组进行Q检验发现,POAG早、中、晚期组与正常对照组相比,球后视神经直径随病程进展逐渐变小,其差异均有统计学意义(P〈0.05),而在视野损害前期,球后视神经直径虽有缩小,但与对照组相比,其差异没有统计学意义(P〉0.05)。结论POAG病人OND大小随病程的进展、视野缺损的加重而缩小;超声测量球后视神经直径是一种简便、准确的检查方法,可帮助疑难青光眼及其它伴有视神经改变的疾病提供诊断依据。Objective The diameters of the retrobulbar optic nerve (OND) in glaucomatous eyes at different stages and in normal eyes were measured by B-scan ultrasonography and the relations between them were analyzed, at the same time the acuity and practicality of this technique were also tested.Methods A total of 115 POAG eyes were included in this study, with 40 normal eyes as the control group.The POAG eyes were divided into 4 groups according to the diagnosis of the standard automated perimetry, 20 eyes in the preeri- metric group (PP), 32 eyes in the early perimetric damage group (EP), 30 eyes in the middle perimetric damage group (MP) and 33 eyes in the late perimetric damage group (LP).The difference between the patients' ages, visual acuities, IOP and OND were also analyzed statistically.Results OND in the normal contrast group was 3.02± 0.35mm.In POAG groups, the OND was 2.96±0.29mm (PP), 2.80±0.41mm (EP), 2.73±0.19mm (MP) and 2.62±0.29mm (LP) respectively.There were significantly statistical difference between normal control eyes and the EP, MP, LP eyes (P〈0.05.While there were no statistical difference between control eyes and the PP eyes (P〉0.05).Conclusions The OND of POAG patients becoming smaller with the progress of the disease under the measurement by ultrasound, which displays a well coherence with the visual field defect.Therefore ultrasound measurement of the OND is a simple and accurate technique to diagnose and stage suspected glaucoma.
