四联手术治疗新生血管性青光眼的疗效分析

摘　　要：目的探讨采取小梁切除联合羊膜植片、虹膜段切除、晶体超乳吸出术治疗新生血管性青光眼的治疗效果。方法回顾性研究20例20只跟新生血管性青光眼，随机分组：实验组10眼行四联手术：对照组10眼行单纯滤过术。常规上方巩膜瓣、小梁切除、上方虹膜节段切除、巩膜瓣下垫压羊膜（商品）、结节缝合巩膜瓣；另采用透明角膜缘切口行超声乳化吸出白内障。手术前测眼压，行前节荧光素造影，统计分析眼压走势、新生血管变化、滤过泡状态。随访3-6个月。结果实验组术眼手术后眼压均低于20mmHg，对照组仅一例1只眼滤过正常，眼压低于20mmHg。四联手术后眼压得到控制，反应轻微，滤过泡形成良好，虹膜新生血管大多消褪，全部眼球得以保留，其中2例保持0．1视力，1例0．2视力。结论四联手术治疗新生血管性青光眼不失为一种有效的可行方法。Objective To observe and evaluate the clinical effect of the four types of joint surgery for neovascular glaucoma. Methods Retrospectively study. Twenty eyes of neovasvular glaucoma were divided into 2 groups randomly, 10 eyes for each group. Experimental group received all four surgery oftrabeculecto- my, cataract surgery, partial iridectomy and amniotic membrane filling; while control group only received trabeculectomy. Results Follow-up 3-6 months. In the experimental group, intraocular pressure lowed than 20mmHg in all the eyes, however, control group only one eye＇s intraocular pressure lowed than 20mmHg. Conclusions The four type of joint surgery are effective treatment methods for neovascular glaucoma.
