摘  要:目的探讨循环抗体在酒精性肝病患者免疫反应中的特异性。方法采用酶联免疫吸附试验(ELISA)检测40例酒精性肝病患者、40例非酒精性肝病患者、30例无肝损害的重度嗜酒者、40例健康对照的血清中循环抗体滴度。结果酒精性肝病患者抗MAAIgG滴度比健康对照组明显增加(平均OD值0.42±0.23、0.10±0.04、P<0.001),非酒精性肝病及重度嗜酒者中抗HSA-MAAIgG抗体(平均OD值0.14±0.08、0.13±0.08)与对照组相比没有显著性差异。酒精性肝病患者抗HSA-MAAIgG值与抗HSA-乙醛(γ=0.643;P<0.0002)和抗HSA-丙醛(γ=0.773;P<0.0001)IgG值呈正相关。结论MAA加合物的循环抗体具有特异性,可引起肝脏自身免疫反应。Objective To explore the specificity of circulating antibodies againstmalondialdehyde -acetaldehyde adducts in the immune response associated with human alcohol - induced liver disease. Methods A MAA adduct detection assay has been developed and used to evaluate MAA antibody specific response . circulating antibodies against MAA adducts were evaluated in 40 patients with alcohol -induced liver disease( ALD), in 40 patients with non alcohol -induced liver disease (NALD), in 30 heavy drinkers (HD)without liver damage and in 40 healthy controls by enzyme - linked immunosorbent assays (ELISA). Results Anti - MAA IgG titers were significantly increased in patients with ALD compared with healthy controls ( mean [ OD ] 0.42 ± 0. 23,0. 10 ± 0.04, P 〈 0. 001 ), Anti - MAA IgG titers in patients with NALD ( mean [ OD ] 0. 14 ± 0. 08 ) and HD( mean[ OD ] 0. 13 ± 0. 08 ) were not significantly different from those of healthy controls. The values of IgG against HAS - MAA in the patients with ALD were positively correlated with those of both anti - HAS - AcA ( y = 0. 643 ; P 〈 0. 0002 ) and anti - HAS - MDA ( y = 0. 773 ;P 〈 0. 0001 ). Conclusions The formation of MAA antigens is specifical during alcohol - induced liver disease and suggest their possible contribution to the development of immunologic reactions.
